集束化综合护理方案预防ICU患者导尿管相关尿路感染的效果研究

Effect of Cluster Comprehensive Nursing Program on Prevention of Catheter Related Urinary Tract Infection in ICU Patients

目的分析集束化综合护理方案预防ICU患者导尿管相关尿路感染的效果。方法选取2020年1月-2021年1月在我院ICU治疗的58例患者为研究对象,采用随机数字表法分为对照组和观察组,各29例。对照组实施常规护理,观察组给予集束化综合护理方案干预,比较两组患者导尿管相关尿路感染发生率、尿管留置时间、膀胱刺激症状持续时间、导尿管不良事件发生率、生活质量评分以及护理满意度。结果观察组导尿管相关尿路感染发生率为6.90%,低于对照组的17.24%,差异有统计学意义(P<0.05);观察组尿管留置时间、膀胱刺激症状持续时间均短于对照组,差异有统计学意义(P<0.05);观察组导尿管不良事件发生率为3.45%,低于对照组的13.79%,差异有统计学意义(P<0.05);观察组情绪角色、身体健康、认知能力、社会功能生活质量评分均高于对照组,差异有统计学意义(P<0.05);观察组护理满意度为93.10%,高于对照组的82.76%,差异有统计学意义(P<0.05)。结论集束化综合护理方案可降低ICU患者导尿管相关尿路感染发生率,缩短导尿管留置时间,减少对膀胱的刺激,预防导尿管相关不良事件,改善患者生活质量,提高护理满意度。

Objective To analyze the effect of cluster comprehensive nursing program on preventing catheter related urinary tract infection in ICU patients.Methods A total of 58 patients treated in ICU of our hospital from January 2020 to January 2021 were selected and randomly divided into control group and observation group,with 29 cases in each group.The control group was given routine nursing,and the observation group was given cluster comprehensive nursing intervention.The incidence of catheter-related urinary tract infection,indwelling time of urinary catheter,duration of bladder irritation symptoms,incidence of catheter adverse events,quality of life score and nursing satisfaction were compared between the two groups.Results The incidence of catheter-related urinary tract infection in the observation group was 6.90%,which was lower than 17.24% in the control group,and the difference was statistically significant(P<0.05).The indwelling time of urinary catheter and the duration of bladder irritation symptoms in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The incidence of catheter adverse events in the observation group was 3.45%,which was lower than 13.79% in the control group,and the difference was statistically significant(P<0.05).The scores of emotional role,physical health,cognitive ability and social function quality of life in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The nursing satisfaction of the observation group was 93.10%,which was higher than 82.76% of the control group,and the difference was statistically significant(P<0.05).Conclusion Cluster comprehensive nursing program can reduce the incidence of catheter related urinary tract infection in ICU patients,shorten the indwelling time of catheter,reduce the stimulation of bladder,prevent catheter related adverse events,improve the quality of life of patients and improve nursing satisfaction.
