摘要
目的了解HIV感染合并结核(Tuberculosis,TB)患者外周血γδT细胞的表型情况,为相关治疗提供依据。方法以HIV感染合并TB患者为病例组(HIV/TB组),以单纯HIV感染者(HIV组)、单纯活动性TB患者(TB组)、健康人群为对照组,分离外周静脉血中单个核细胞,通过流式细胞术检测γδT细胞不同亚群的绝对计数和百分比以及其表达CD4、CD8情况。结果对于Vδ_(2)/CD3和Vδ_(2)T细胞绝对计数,HIV组及TB组均显著低于健康对照组(P=0.00),且HIV/TB组Vδ_(2)T细胞绝对计数较HIV组及TB组显著降低(P=0.00)。与健康对照组相比,TB组Vδ_(1)/CD3和Vδ_(1)T细胞绝对计数差异均无统计学意义,但HIV组Vδ_(1)/CD3显著升高(P=0.03)。HIV/TB组的Vδ_(2)/Vδ_(1)与TB组相比显著降低(P=0.00)。HIV组和健康对照组的CD4^(+)CD8^(+)Vδ_(2)/Vδ_(2)差异无统计学意义(P=0.80)。CD4^(+)CD8^(+)Vδ_(2)/Vδ_(2)在HIV/TB组、HIV组及TB组均显著升高,且HIV/TB组高于HIV组和TB组。结论单一HIV或TB感染均改变了γδT细胞亚群之间的平衡,尤其是Vδ_(2)T细胞数量显著降低。而在两种病原体共同感染时,进一步增加了Vδ_(2)T细胞的耗竭。此外,CD4^(+)CD8^(+)Vδ_(2)T细胞可能在HIV及TB感染免疫中发挥一定的作用。
Objective To understand the subtype ofγδT cells in HIV-positive patients with active tuberculosis(TB),thereby providing a reference for related treatments.Methods There were three patient groups in our study including HIV-infected patients co-infected with TB were the experimental group(HIV/TB group),HIV-infected patients(HIV group),and active TB patients(TB group).Healthy subjects were the control groups.The absolute count,the percentage,and the expression ofγδT of CD4 or CD8 cells in peripheral blood mononuclear cells from these patients were detected by the flow cytometry method.Results The absolute number and the percentage of Vδ_(2)T cells(Vδ_(2)/CD3)in the HIV or TB group were lower than healthy controls(P=0.00)and the figure in the HIV/TB group reduced more markedly than that in the HIV or TB group(P=0.00).There was no significant difference in the absolute number and the percentage of Vδ_(2)T cells(Vδ_(1)/CD3)between the TB group and healthy controls,but Vδ_(1)/CD3 in the HIV group was significantly higher than that in healthy controls(P=0.03).Vδ_(2)/Vδ_(1)was significantly decreased in HIV/TB group compared with the TB group(P=0.00).There were no significant differences in the percentage of CD4^(+)CD8^(+)Vδ_(2)T/Vδ_(2) cells between the HIV group and healthy controls(P=0.80).The percentage of CD4^(+)CD8^(+)Vδ_(2)T/Vδ_(2) cells of the HIV/TB group was significantly higher than that in healthy controls and higher than that in the HIV group and the TB group.Conclusions HIV or TB infection could change the balance between theγδT cells subgroups,especially making the number of Vδ_(2)T cells decline.The subgroup showed a more serious depletion in the co-infection group.CD4^(+)CD8^(+)Vδ_(2)T cells may play a certain role in immune responses when people are co-infected with HIV and TB.
