IMB理念引导超早期精细化康复功能锻炼结合局部加压冷疗在膝关节置换术后早期功能恢复中的应用

Application of information motivation behavior skills concept-guided ultra-early fine rehabilitation functional exercise combined with local pressure cryotherapy in early functional recovery after knee arthroplasty

目的探讨以信息-动机-行为技巧(IMB)理念引导超早期精细化康复功能锻炼结合局部加压冷疗在膝关节置换术后早期功能恢复中的应用效果。方法选择2019年12月至2020年12月期间延安市中医医院收治的56例膝关节置换术患者为研究对象,根据随机数表法将患者分为研究组和对照组各28例。对照组患者采用常规功能锻炼,研究组患者采用IMB理念引导超早期精细化康复功能锻炼结合局部加压冷疗。两组患者均连续干预1个月后,记录并比较干预前后患者的疼痛(VAS)评分、肿胀值、膝关节功能(HSS)评分及临床疗效优良率。结果干预前,两组患者的VAS评分、肿胀值、HSS总评分比较差异均无统计学意义(P>0.05),干预后,两组患者的VAS评分、肿胀值均明显降低,且研究组患者的VAS评分、肿胀值分别为(2.65±0.78)分、(0.82±0.27)cm,明显低于对照组的(4.49±0.83)分、(1.64±0.44)cm,差异均有统计学意义(P<0.05),两组患者的HSS总评分均明显升高,且研究组患者的HSS总评分为(79.63±10.56)分,明显高于对照组的(68.41±11.42)分,差异有统计学意义(P<0.05);研究组患者的临床疗效优良率为96.43%,明显高于对照组的71.43%,差异有统计学意义(P<0.05)。结论膝关节置换术后给予患者IMB理念引导超早期精细化康复功能锻炼结合局部加压冷疗,能够改善患者的疼痛及肿胀程度,有助于膝关节早期功能恢复,具有临床应用价值。

Objective To explore the application effect of ultra-early fine rehabilitation functional exercise guided by the concept of information motivation behavior skills(IMB)combined with local pressure cryotherapy in the early functional recovery after knee arthroplasty.Methods Fifty-six patients with knee arthroplasty treated in Yan'an Traditional Chinese Medicine Hospital from December 2019 to December 2020 were selected as research subjects.According to random number table,the patients were divided into the study group and the control group,with 28 patients in each group.The patients in the control group applied routine functional exercise,and the patients in the study group used IMB concept-guided ultra-early fine rehabilitation functional exercise combined with local pressure cryotherapy.After one month of continuous intervention,the pain(VAS)score,swelling value,knee function(HSS)score,and the excellent and good rate of clinical curative effect were recorded and compared before and after intervention.Results Before intervention,there was no significant difference in VAS score,swelling value and total HSS score between the two groups(P>0.05);after intervention,the VAS score and swelling value of the two groups decreased significantly,and the VAS score and swelling value of the study group were(2.65±0.78)points and(0.82±0.27)cm,which were significantly lower than(4.49±0.83)points and(1.64±0.44)cm of the control group(P<0.05);the total score of HSS in the study group was(79.63±10.56)points,which was significantly higher than(68.41±11.42)points in the control group(P<0.05);the excellent and good rate of clinical efficacy in the study group was 96.43%,which was significantly higher than 71.43% in the control group(P<0.05).Conclusion After knee arthroplasty,IMB concept-guided ultra-early fine rehabilitation functional exercise combined with local pressure cryotherapy can improve the degree of pain and swelling,contribute to the early functional recovery of knee joint,and have clinical application value.
