超声心动图对房颤患者左室收缩功能的诊断价值分析 被引量:1

摘要 目的 分析超声心动图对心房颤动(房颤)患者左室收缩功能的诊断价值。方法 选择60例房颤患者作为观察组,并选择同期进行体检的60例健康者作为对照组。对比两组各项超声参数以及有无左心房自发性显影(LASEC)房颤患者的各项超声参数。结果 观察组的左心房前后径及左右径大于对照组,左心耳面积变化率、血流最大排空速度(Lev)、最大充盈速度(Lfv)低于对照组,差异具有统计学意义(P<0.05)。60例房颤患者当中,有LASEC的患者22例(36.67%),无LASEC的患者38例(63.33%)。有LASEC的患者左心房前后径为(39.72±3.34)mm、左心房左右径为(48.87±2.89)mm、左心耳面积变化率为(29.63±6.84)%、左心耳Lev为(17.42±5.73)cm/s、左心耳Lfv为(17.73±7.58)cm/s;无LASEC的患者左心房前后径为(34.87±3.64)mm、左心房左右径为(41.75±3.24)cm、左心耳面积变化率为(34.46±4.83)%、左心耳Lev为(20.94±5.13)cm/s、左心耳Lfv为(23.35±4.89)cm/s。有LASEC患者的左心房前后径和左右径大于无LASEC患者,左心耳面积变化率、Lev、Lfv低于无LASEC患者,差异具有统计学意义(P<0.05)。结论 超声心动图用于检测房颤患者左室收缩功能的临床应用当中,能够明确分析患者左心房以及左心耳各项临床指标的变化情况,值得临床推广。 Objective To analyze the diagnostic value of echocardiography in detecting systolic function in patients with atrial fibrillation.Methods 60 patients with atrial fibrillation were selected as the observation group,and 60 healthy patients who came to the hospital for physical examination during the same period were selected as the control group.All ultrasound parameters were compared between the two groups as well as in patients with atrial fibrillation with or without left atrial spontaneous echo contrast(LASEC).Results The anteroposterior and left-right diameters of the left atrium of the observation group were greater than those of the control group;the change rate of the left atrial appendage area,maximum emptying velocity(Lev)and maximum filling velocity(Lfv)of the left atrial appendage were lower than those of the control group;All the differences were statistically significant(P<0.05).Among the 60 patients with atrial fibrillation,22 cases(36.67%)had LASEC and 38 cases(63.33%)had no LASEC.In patients with LASEC,the anteroposterior diameter of the left atrium was(39.72±3.34)mm,the left-right diameters of the left atrium was(48.87±2.89)mm,the change rate of the left atrial appendage area was(29.63±6.84)%,and the Lev of the left atrial appendage was(17.42±5.73)cm/s,Lfv of left atrial appendage was(17.73±7.58)cm/s.In patients without LASEC,the anteroposterior diameter of the left atrium was(34.87±3.64)mm,the left-right diameters of the left atrium was(41.75±3.24)cm,the change rate of the left atrial appendage area was(34.46±4.83)%,and the Lev of the left atrial appendage was(20.94±5.13)cm/s,Lfv of left atrial appendage was(23.35±4.89)cm/s.The anteroposterior and left-right diameters of the left atrium in patients with LASEC were larger than those without LASEC,and the change rate of left atrial appendage area,Lev and Lfv of the left atrial appendage were lower than those without LASEC.All the differences were statistically significant(P<0.05).Conclusion In the detection of left ventricular systolic function of patients with atrial fibrillation,chocardiography can clearly analyze the changes of various clinical indicators of the left atrium and left atrial appendage of patients,and it is worthy of clinical promotion.
