鼻内镜手术联合布地奈德治疗慢性鼻窦炎的效果观察

Effect of endoscopic sinus surgery combined with budesonide in the treatment of chronic sinusitis

目的 探讨鼻内镜手术联合布地奈德治疗慢性鼻窦炎的临床效果。方法 68例慢性鼻窦炎患者,随机分为对照组与观察组,各34例。对照组采用鼻内镜手术治疗,观察组在对照组基础上应用布地奈德治疗。比较两组治疗效果及治疗后并发症发生情况、复发情况、主观症状评分。结果 观察组总有效率为91.2%,高于对照组的70.6%,差异有统计学意义(P<0.05)。观察组鼻塞、嗅觉障碍、面部疼痛、流涕评分分别为(1.9±0.4)、(2.5±0.2)、(1.1±0.3)、(1.7±0.4)分,均显著低于对照组的(2.7±0.5)、(3.3±0.6)、(1.8±0.2)、(2.4±0.1)分,差异具有统计学意义(P<0.05)。观察组治疗后并发症发生率与复发率均低于对照组,差异具有统计学意义(P<0.05)。结论 鼻内镜手术联合布地奈德治疗慢性鼻窦炎,能有效改善临床症状,促进术后恢复,并发症少,复发率低,比单纯手术治疗效果更好,值得临床推广。

Objective To discuss the clinical effect of endoscopic sinus surgery combined with budesonide in the treatment of chronic sinusitis.Methods A total of 68 patients with chronic sinusitis were randomly divided into control group and observation group,with 34 cases in each group.The control group was treated with endoscopic sinus surgery,and the observation group was treated with budesonide on the basis of the control group.The therapeutic effect,complications,recurrence and subjective symptom scores after treatment were compared between the two groups.Results The total effective rate of the observation group was 91.2%,which was higher than 70.6%of the control group,and the difference was statistically significant(P<0.05).The scores of nasal congestion,olfactory dysfunction,facial pain,and runny nose in the observation group were(1.9±0.4),(2.5±0.2),(1.1±0.3),and(1.7±0.4)points,which were significantly lower than(2.7±0.5),(3.3±0.6),(1.8±0.2),and(2.4±0.1)points of the control group,and the difference was statistically significant(P<0.05).The complication rate and recurrence rate after treatment in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Endoscopic sinus surgery in combination with budesonide is more effective than surgery alone in the treatment of chronic sinusitis.It can effectively improve clinical symptoms and promote postoperative recovery,with fewer complications and low recurrence rate.It is worthy of clinical promotion.
