溃疡性结肠炎患者降钙素原、红细胞沉降率、C-反应蛋白水平分析

Relationship between procalcitonin,erythrocyte sedimentation rate,C-reactive protein and endoscopic severity of ulcerative colitis

目的探讨溃疡性结肠炎(UC)患者降钙素原、红细胞沉降率、C-反应蛋白水平与内镜下严重程度的关系。方法选择2019年5月—2020年12月64例行内镜检查的UC患者,根据Mayo肠镜评分分为轻度组27例、中度组22例、重度组15例,比较3组患者实验室指标水平,分析实验室指标与内镜下严重程度的关系。结果中度组和重度组患者D-二聚体、纤维蛋白原、血小板计数、红细胞沉降率、降钙素原、C-反应蛋白水平明显高于轻度组(P<0.05),且重度组明显高于中度组(P<0.05);D-二聚体、纤维蛋白原、血小板计数与内镜下严重程度无显著相关性(r值分别为0.171、0.185、0.085,P>0.05),红细胞沉降率、C-反应蛋白、降钙素原与内镜下严重程度呈正相关关系(r值分别为0.511、0.654、0.689,P<0.01);红细胞沉降率、C-反应蛋白、降钙素原评估中重度UC的AUC分别为0.750、0.725、0.828。结论降钙素原、红细胞沉降率、C-反应蛋白与UC内镜下严重程度密切相关,在评估UC患者肠道情况方面有一定价值。

Objective To investigate the relationship between procalcitonin,erythrocyte sedimentation rate,C-reactive protein and endoscopic severity of ulcerative colitis(UC).Methods A total of 64 UC patients admitted from May 2019 to December 2020 were selected as the observation objects.All patients underwent endoscopy,they were divided into mild group(27 cases),moderate group(22 cases)and severe group(15 cases)according to Mayo colonoscopy score.The relationship between laboratory indicators and endoscopic severity was analyzed.Results The levels of D-D,fibrinogen,platelet count,erythrocyte sedimentation rate,procalcitonin and C-reactive protein in moderate and severe group were significantly higher than those in mild group(P<0.05),and those in severe group were significantly higher than those in moderate group(P<0.05).D-D,fibrinogen and platelet count were not significantly correlated with endoscopic severity(r=0.171,r=0.185,r=0.085,P>0.05),but erythrocytic sedimentation rate,C-reactive protein and procalcitonin were positively correlated with endoscopic severity(r=0.511,r=0.654,r=0.689,P<0.01).The AUC of erythrocyte sedimentation rate,C-reactive protein and procalcitonin in the evaluation of moderate and severe UC were 0.750,0.725 and 0.828.Conclusion Procalcitonin,erythrocyte sedimentation rate and C-reactive protein are closely related to endoscopic severity,and have certain value in evaluating intestinal conditions of UC patients.
