2型糖尿病患者血清镁、糖化白蛋白与增殖性糖尿病性视网膜病变发生的相关性分析 被引量:5

Relationship of Proliferative Diabetic Retinopathy with Serum Magnesium and Glycosylated Albumin in Patients with Type 2 Diabetes Mellitus

摘要 【目的】探讨2型糖尿病(T2DM)患者血清镁和糖化白蛋白水平与增殖性糖尿病性视网膜病变(PDR)发生的相关性。【方法】选择2017年10月至2018年12月本院诊治的T2DM患者90例,根据是否发生PDR分为PDR组(42例)和非增殖性糖尿病性视网膜病变(NPDR)组(48例)。采用免疫透射比浊法检测两组患者血清糖化白蛋白水平,采用酶联免疫吸附法检测血清镁水平,并采用受试者工作曲线(ROC)分析血清镁和糖化白蛋白对T2DM伴PDR的诊断效能,用Logistic回归分析影响T2DM伴PDR发生的危险因素。【结果】与NPDR组相比,PDR组血清镁水平较低(P<0.05),糖化白蛋白水平较高(P<0.05)。ROC曲线显示:血清镁、糖化白蛋白诊断T2DM伴PDR的曲线下面积(AUC)分别为0.699、0.768,截断值分别为0.79 mmol/L、16.35%;两者联合诊断的AUC为0.857,明显高于血清镁和糖化白蛋白的单独诊断(P<0.05),敏感度及特异度分别为71.4%、87.5%。血清镁、糖化白蛋白、甘油三酯(TG)、病程是影响T2DM伴PDR发生的危险因素(P<0.05)。【结论】T2DM伴PDR患者血清镁明显降低,糖化白蛋白则异常升高,且二者是影响T2DM伴PDR发生的危险因素,可作为诊断T2DM伴PDR的潜在指标。 【Objective】To investigate the relationship of proliferative diabetic retinopathy(PDR)with serum magnesium and glycosylated albumin in patients with type 2 diabetes mellitus(T2DM).【Methods】A total of 90 patients with T2DM in our hospital from October 2017 to December 2018 were selected and divided into the proliferative diabetic retinopathy(PDR)group(n=42 cases)and non-proliferative diabetic retinopathy(NPDR)group(n=48 cases)according to the occurrence of PDR or not.The level of serum glycosylated albumin was detected by immunoturbidimetry and the level of serum magnesium was measured by enzyme-linked immunosorbent assay.The ROC curve was used to analyze the diagnostic efficacy of serum magnesium and glycosylated albumin in patients with T2DM complicated with PDR.In addition,logistic regression analysis was used to analyze the risk factors of PDR in patients with T2DM.【Results】Compared with the NPDR group,the serum magnesium level in the group with PDR was lower(P<0.05),and the glycosylated albumin level was higher(P<0.05).The ROC curve showed that the area under curve(AUC)of serum magnesium and glycosylated albumin in the diagnosis of PDR in patients with T2DM were 0.699 and 0.768,their cut-off values were 0.79 mmol/L and 16.35%,respectively.The AUC of combined diagnosis of magnesium and glycosylated albumin was 0.857,which was significantly higher than that of serum magnesium and glycosylated albumin alone(P<0.05),the sensitivity and specificity were 71.4%and 87.5%,respectively.Serum magnesium,glycosylated albumin,triglyceride(TG),course of disease were the risk factors of PDR in patients with diabetes mellitus(P<0.05).【Conclusion】The serum magnesium of patients with T2DM with PDR is significantly reduced,while glycosylated albumin is abnormally elevated,and both are risk factors for T2DM with PDR,which can be used as potential indicators for later disease diagnosis.
