德谷门冬双胰岛素治疗2型糖尿病的短期疗效观察 被引量:9

Observation on the short-term curative effect of insulin degludec/insulin aspart in the treatment of type 2 diabetes mellitus

摘要 目的 观察德谷门冬双胰岛素治疗2型糖尿病(T2DM)的疗效及安全性。方法 选取2020年6月-2021年6月兰州大学第二医院门诊/住院诊治的经口服降糖药控制不佳的T2DM患者80例,随机分为2组(双相组和对照组),每组各40例。分别于治疗后12、26周复查,检测指标包括空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)等。结果 双相组FPG、2hPG、HbA1c下降幅度明显高于对照组(P<0.05),但双相组不同注射时间点的下降幅度相似;双相组总体及夜间低血糖发生率较对照组更低(P<0.05),且2组患者体重增加相似(P> 0.05)。治疗12周后,双相组胰岛素日剂量少于对照组(P <0.05);治疗26周后,双相组FPG达标率高于对照组(P <0.05)。结结论 与甘精胰岛素相比,德谷门冬双胰岛素的降糖效果更强,血糖达标率更高,低血糖发生率更低,且不引起体重明显变化。 Objective To observe the efficacy and safety of insulin degludec/insulin aspart in the treatment of type 2 diabetes mellitus(T2 DM). Methods From June 2020 to June 2021, 80 T2 DM patients with poorly controlled oral antidiabetic drugs who were diagnosed and treated in the Second Hospital of Lanzhou University were selected and randomly divided into two groups, with 40 cases in each group. Re-examination was conducted at 12 weeks and 26 weeks respectively. The detection indicators included fasting blood glucose(FPG), 2 hours postprandial blood glucose(2 hPG) and glycosylated hemoglobin(HbA1 c), et al. Results The decline of FPG, 2 hPG, HbA1 c in the biphasic group was significantly higher than that in the control group(P < 0.05), but the decrease of FPG, 2 hPG, HbA1 c at different injection time points in the biphasic group was similar. The overall and nocturnal hypoglycemia rates of the biphasic group were lower than those of the control group(P <0.05), and the increase in body weight of the two groups was similar(P > 0.05). After 12 weeks of treatment,the daily dose of insulin in the biphasic group was less than that in the control group(P < 0.05);after 26 weeks, the FPG compliance rate of the biphasic group was higher than that of the control group(P < 0.05), and the daily insulin doses of the two groups were similar(P > 0.05). Conclusion Compared with insulin glargine,the insulin degludec/insulin aspart has a stronger hypoglycemic effect, with a higher rate of blood glucose compliance and a lower incidence of hypoglycemia, without causing significant changes in body weight.
