丹栀逍遥丸辅治多囊卵巢综合征排卵障碍性不孕症的疗效及对子宫内膜容受性影响

Efficacy of Danzhi Xiaoyao pills in the adjunctive treatment of infertility due to ovulation dysfunction caused by polycystic ovary syndrome and its influence on endometrial receptivity

目的:探讨丹栀逍遥丸辅治多囊卵巢综合征(PCOS)排卵障碍性不孕症的疗效及对子宫内膜容受性的影响。方法:选取2018年2月至2020年10月常州市中医医院收治的86例PCOS排卵障碍性不孕症患者为研究对象进行前瞻性研究,以随机数表法分为研究组、对照组,每组各43例。对照组行常规西医治疗,研究组在对照组的基础上给予丹栀逍遥丸治疗。对比2组临床疗效、性激素水平、子宫内膜容受性,记录2组周期排卵、临床妊娠及药物安全性情况。结果:研究组总有效率高于对照组(P<0.05)。2组治疗后的睾酮(T)、促黄体生成素(LH)水平均低于治疗前(P<0.05),雌二醇(E2)、促卵泡生成激素(FSH)水平均高于治疗前(P<0.05);研究组治疗后的T、LH水平均低于对照组(P<0.05),E2、FSH水平均高于对照组(P<0.05)。2组治疗后的子宫内膜厚度(Em)水平均高于治疗前(P<0.05),子宫内膜螺旋动脉搏动指数(PI)、阻力指数(RI)水平均低于治疗前(P<0.05);研究组治疗后的Em水平高于对照组(P<0.05),PI、RI水平均低于对照组(P<0.05)。2组周期排卵率对比差异无统计学意义(P>0.05),研究组妊娠率高于对照组(P<0.05)。2组总不良反应发生率对比差异无统计学意义(P>0.05)。结论:丹栀逍遥丸辅治PCOS排卵障碍性不孕症可明显改善性激素水平及子宫内膜容受性,进而增强治疗效果,提高妊娠率,且安全性良好。

OBJECTIVE To explore the effect of Danzhi Xiaoyao pills in the treatment of polycystic ovary syndrome(PCOS)infertility due to ovulation dysfunction and its influence on endometrial receptivity.METHODS Totally 86 patients with PCOS ovulation dysfunction infertility admitted to Changzhou Hospital of Traditional Chinese Medicine from February 2018 to October 2020 were selected as research subjects for prospective research.They were divided into study group and control group by random number table method with43 cases in each group.The control group was received conventional western medicine treatment,and the study group was given Danzhi Xiaoyao pills on the basis of the control group.The clinical efficacy,sex hormone levels and endometrial receptivity were compared between the two groups.The ovulation,clinical pregnancy and drug safety of the two groups were recorded.RESULTS The total effective rate of the study group was higher than that of the control group(P<0.05).The levels of testosterone(T)and luteinizing hormone(LH)after treatment in the two groups were lower than those before treatment(P<0.05),and the levels of estradiol(E2)and follicle stimulating hormone(FSH)were higher than those before treatment(P<0.05).T and LH levels of the study group after treatment were lower than those of the control group(P<0.05),and E2 and FSH levels were higher than those of the control group(P<0.05).The endometrial thickness(Em)of the two groups after treatment was higher than that before treatment(P<0.05),and the endometrial spiral artery pulsation index(PI)and resistance index(RI)levels were lower than those before treatment(P<0.05)).Em levels in the study group after treatment were higher than those of the control group(P<0.05),and PI and RI levels were lower than those of the control group(P<0.05).There was no significant difference in ovulation rate between the two groups(P>0.05),and the pregnancy rate in the study group was higher than that in the control group(P<0.05).There was no statistically significant difference in the incidence of total adverse reactions between the two groups(P>0.05).CONCLUSION Danzhi Xiaoyao pills in the adjunctive treatment of PCOS ovulation dysfunction infertility can significantly improve the levels of sex hormones and endometrial receptivity,thereby enhancing the therapeutic effect and increasing the pregnancy rate with good safety.
