摘要
目的基于圆形分布法探讨广州地区孕妇单胎妊娠发生早产的季节及月份分布规律。方法将统计年度看成1个圆周,用圆形分布法对广州医科大学附属第三医院2009年1月1日至2017年12月31日之间早产发生的季节及月份进行聚集性分析。结果2009年至2017年在广州医科大学附属第三医院单胎分娩孕妇共47963例,其中早产5639例(11.76%),早产中未足月胎膜早破1682例(3.51%),不明原因早产784例(1.63%);早产及不明原因早产的发生高峰有时间聚集性,发生高峰均在秋季(平均角α分别为29.98、18.17,Z>Z_(0.05),P<0.05),高峰日分别在9月29日及9月16日(平均角α分别为84.48、72.61,Z>Z_(0.05),P<0.05),未足月胎膜早破早产发生高峰未显示出时间聚集性(平均角α分别为15.64、67.03,Z<Z_(0.05),P>0.05)。结论早产及不明原因早产的发生高峰有时间聚集性,均在秋季,高峰日在9月中下旬。秋季应警惕早产及不明原因早产,加强对存在早产危险因素孕妇的监护,合理安排医疗资源,改善新生儿结局。
Objective To investigate distribution rules of season and month of preterm birth in singleton pregnant women in Guangzhou city.Methods A statistical year of preterm birth was regarded as a circle.The season and month clustering of preterm infants who gave a birth in The Third Affiliated Hospital of Guangzhou Medical University from January 1 st,2009 to December 31 th,2017 were analyzed by circular distribution algorithm.Results A total of 47963 women with singleton pregnacy delivered in the Third Affiliated Hospital of Guangzhou Medical University over a period from 2009 to 2017,including 5639 cases of preterm birth(11.76%).Of them,1682 cases of preterm birth were caused by preterm premature rupture of fetal membranes(PPROM)(3.51%),and 784 cases of preterm birth were caused by unexplained reasons(1.63%).The occurerence peaks of the preterm birth and unexplained preterm bith both shown a time aggregation,which were in autumn(mean angle α=9.98 and 18.17 respectively,Z>Z_(0.05),both P<0.05),and their peak days were on September 29 and September 16(mean angle α=94.48 and 72.61 respectively,Z>Z_(0.05),both P<0.05).While the occurerence peak of preterm birth caused by PPROM shown no time aggregation(mean angle α=15.64 and 67.03 respectively,Z<Z_(0.05),both P>0.05).Conclusion Preterm birth and unexplained preterm birth occur with a time aggregation,which centralize in mid to late September.The people should be vigilant to preterm birth and unexplained preterm birth in autumn,and monitoring of those pregnant women with risk factors of preterm birth should be strengthened,and medical resources should be reasonably arranged to improve neonatal outcomes.
