微创单孔胸腔镜解剖性肺段切除术治疗早期非小细胞肺癌临床分析

Clinical analysis of minimally invasive single-port thoracoscopic anatomical segmentectomy for early non-small cell lung cancer

目的探讨胸腔镜解剖性肺段切除术的安全性及可行性.方法对我院2014年1月至2019年12月行胸腔镜解剖性肺段切除术的12例患者临床资料进行回顾性分析,结合术中、术后及近期预后相关参数,并结合相关文献对解剖性肺段切除术的现况、优势及术中操作要点进行分析.结果本组12例患者均采用单孔胸腔镜技术完成解剖性肺段切除手术,术中无中转开胸手术病例,术后无二次手术及死亡病例;术后出现肺部感染1例,胸腔积液1例,气胸2例,经相应治疗均痊愈出院.术后随访6个月,12例患者肺功能指标减少比率不明显,未出现复发及死亡病例.结论采用单孔胸腔镜技术进行解剖性肺段切除术创伤小、恢复快、术后住院时间短,术后并发症少,是一种安全、可行的治疗方法,但其技术难度大、手术操作复杂,对手术团队的整体配合有较高要求.

Objective To explore the safety and feasibility of thoracoscopic anatomical segmentectomy.Methods The clinical data of 12 patients who underwent thoracoscopic anatomical segmentectomy in our hospital from January 2014 to December 2019 were retrospectively analyzed,combined with intraoperative,postoperative and short-term prognostic parameters,and combined with relevant literature.The current status,advantages and main points of intraoperative operation of anatomical segmentectomy are ana-lyzed.Results All 12 patients in this group were completed anatomical lung resection using single-port thoracoscopy.There were no cases of conversion to thoracotomy during the operation,no secondary operations or death cases after the operation;follow-up for 6 months,12 patients The reduction rate of lung function indexes was not obvious,and there were no cases of recurrence and death.Conclusion The use of single-port thoracoscopic technique for anatomical segmentectomy is a safe and feasible treatment method with less trauma,quick recovery,short postoperative hospital stay,and few postoperative complications,but it is technically difficult and complicated and has high requirements for the overall cooperation of the surgical team.
