右美托咪定对小儿七氟醚面罩吸入麻醉苏醒期躁动的影响

Effect of Dexmedetomidine on Emergence Agitation after Sevoflurane Mask Inhalation Anesthesia in Children

目的:分析持续静脉泵注右美托咪定对小儿七氟醚面罩吸入麻醉苏醒期躁动的影响。方法:选取2018年7月至2020年6月于佛山市顺德区北滘医院行外科手术的50例患儿,分为观察组(25例)与对照组(25例)。对照组患儿行持续静脉泵注0.9%氯化钠注射液,观察组患儿持续静脉泵注右美托咪定。比较两组患儿拔管后心率(HR)、平均动脉压(MAP)、麻醉苏醒期躁动评分〔小儿麻醉后苏醒期躁动(PAED)评分〕、各项术后时间指标、以及不良反应。结果:观察组患儿HR、MAP水平及PAED评分均显著低于对照组,差异具有统计学意义(P<0.05)。观察组患儿苏醒时间、拔管时间、送离麻醉后监测治疗室(PACU)时间均显著短于对照组,差异具有统计学意义(P<0.05)。观察组患儿不良反应发生率低于对照组,差异具有统计学意义(P<0.05)。结论:患儿采用右美托咪定持续泵注能有效降低患儿经七氟醚麻醉后苏醒躁动发生率,缩短患儿拔罐时间,提高对患儿治疗的安全性。

Objective To analyze the effect of continuous intravenous pumping of dexmedetomidine on emergence agitation after sevoflurane mask inhalation anesthesia in children.Methods 50 children undergoing surgical operations in Shunde District Beijiao Hospital of Foshan city from July 2018 to June 2020 were selected and divided into observation group and control group with 25 cases in each group.The control group received continuous intravenous pumping of 0.9% sodium chloride injection,and the observation group received continuous intravenous pumping of dexmedetomidine.The heart rate(HR),mean arterial pressure(MAP),pediatric anesthesia emergence delirium(PAED)score,postoperative time indicators,and adverse reactions were compared between the two groups.Results HR,MAP and PAED score in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The awakening time,extubation time and postanesthesia care unit(PACU)time of the observation group were significantly shorter than those of the control group,with statistical significance(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Continuous pumping with dexmedetomidine can effectively reduce the incidence of agitation after sevoflurane anesthesia in children,shorten the cupping time,and improve the safety of treatment for children.
