桃红四物汤合五味消毒饮对四肢骨折患者肢体肿胀、骨折愈合时间的影响

The effect of Tao-hong-si-wu decoction combined with Wu-wei-xiao-du decoction on limb swelling and fracture healing time in patients with limb fractures

目的探讨桃红四物汤合五味消毒饮对四肢骨折患者肢体肿胀、骨折愈合时间的影响。方法2017年7月至2018年7月我院收治的四肢骨折患者60例,按照随机数字表法分为观察组和对照组各30例,均行手术治疗,对照组术后给予常规预防感染、消炎止痛、制动等,观察组在对照组基础上内服桃红四物汤合五味消毒饮,比较两组肢体肿胀评分、骨折愈合时间、血清炎症因子C-反应蛋白(CRP)、肿瘤坏死因子(TNF-ɑ)、白介素-6(IL-6)水平、视觉模拟疼痛评分(VAS)及关节活动度(ROM)。结果术后14 d,两组肿胀程度评分均减少,且观察组评分低于对照组(P<0.05),观察组肢体肿胀恢复时间、骨折愈合时间低于对照组(P<0.05);观察组术后14 d的血清CRP、TNF-ɑ、IL-6水平均低于对照组(P<0.05);术后3、7、14 d,观察组VAS评分均低于对照组,ROM大于对照组(P<0.05)。结论桃红四物汤与五味消毒饮联合治疗术后四肢骨折患者更能促进肢体肿胀恢复进程,提高骨折愈合效率,且有效降低患者术后炎症反应和疼痛程度,促进患者术后关节活动的恢复,值得推广。

Objective To investigate the effect of Tao-hong-si-wu decoction combined with Wu-wei-xiao-du decoction on limb swelling and fracture healing time in patients with limb fractures.Methods Sixty patients with limb fractures admitted to our hospital between July 2017 and July 2018 were enrolled in this study.The patients were divided into an observation group and a control group by using the random number table method,30 in each group.All patients were treated with surgery.The control group was given routine drug treatment for infection prevention,anti-inflammatory and pain relief,immobilization,etc.On this basis,the observation group orally took Tao-hong-si-wu decoction combined with Wu-wei-xiao-du decoction.The situation of limb swelling,fracture healing time,changes in the levels of serum inflammatory factors such as C-reactive protein(CRP),tumor necrosis factor(TNF-ɑ),and interleukin-6(IL-6)before and after surgery,pain visual analogue score(VAS)and joint range of motion(ROM)were compared between the two groups.Results After 14 days of surgery,the swelling degree scores were reduced in both groups,and the swelling degree scores in the observation group were significantly milder than those in the control group(P<0.05).The recovery time of limb swelling and fracture healing time of the observation group were significantly shorter than those of the control group(P<0.05).The serum CRP,TNF-ɑand IL-6 levels of the observation group after 14 days of surgery were significantly lower than those of the control group(P<0.05).After 3,7 and 14 days of surgery,the VAS scores of the observation group were significantly lower than those of the control group,while ROM was significantly larger than that of the control group(P<0.05).Conclusion Tao-hong-si-wu decoction combined with Wu-wei-xiao-du decoction for treating patients with limb fractures can promote the recovery of limb swelling,improve the fracture healing efficiency,effectively reduce the postoperative inflammation and pain,and promote the recovery of joint activities after surgery.It is worth promoting.
