小剂量多巴胺联合磷酸肌酸钠对肺炎并发心力衰竭患儿IGF-1、D-dimer、cTnI、PCT水平的影响

Effect of Low Dose Dopamine Combined with Creatine Phosphate Sodium on Levels of IGF-1, D-dimer, cTnI and PCT in Children with Pneumonia Complicated with Heart Failure

目的:评估小剂量多巴胺联合磷酸肌酸钠治疗肺炎并发心力衰竭患儿的效果及对胰岛素样生长因子-1(IGF-1)、D-二聚体(D-dimer)、心肌肌钙蛋白I(cTnI)、血清降钙素原(PCT)水平的影响。方法:以2020年1月-2021年1月抚顺市中医院收治的102例肺炎并发心力衰竭患儿作为研究对象,采用随机抽签法分为观察组和对照组,各51例。对照组予以磷酸肌酸钠治疗,观察组予以小剂量多巴胺联合磷酸肌酸钠治疗。比较两组患儿临床疗效、心功能指标、IGF-1、D-dimer、cTnI、PCT水平及不良反应发生率。结果:观察组总有效率为90.20%,高于对照组的74.51%(P<0.05)。治疗后观察组患儿的左室射血分数(LVEF)、血氧饱和度(SpO;)均高于对照组,心率(HR)低于对照组(P<0.05)。治疗后观察组IGF-1、D-dimer、cTnI、PCT均低于对照组(P<0.05)。两组患儿治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论:应用小剂量多巴胺联合磷酸肌酸钠治疗肺炎并发心力衰竭患儿效果良好,患儿的心功能各项指标及IGF-1、D-dimer、cTnI、PCT指标水平均有改善,治疗安全性较好,可临床推广。

Objective:To evaluate the effect of low dose Dopamine combined with Creatine Phosphate Sodium in the treatment of pneumonia complicated with heart failure in children and the influence on levels of IGF-1,D-dimer,cTnI and PCT.Method:A total of 102 cases of pneumonia complicated with heart failure in Fushun Hospital of Traditional Chinese Medicine from January 2020 to January 2021 were randomly divided into observation group and control group,with 51 cases in each group.The control group was treated with Creatine Phosphate Sodium,and the observation group was treated with low dose Dopamine combined with Creatine Phosphate Sodium.The clinical efficacy,cardiac function index,IGF-1,D-dimer,cTnI,PCT index levels and the incidence of adverse reactions were compared between the two groups.Result:The total effective rate of the observation group was 90.20%,which was higher than 74.51% of the control group (P<0.05).After treatment,LVEF and SpO;of the observation group were higher than those of the control group,and HR of the observation group was lower than that of the control group (P<0.05).After treatment,IGF-1,D-dimer,cTnI and PCT in the observation group were lower than those in the control group (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion:The application of low dose Dopamine combined with Creatine Phosphate Sodium in the treatment of children with pneumonia complicated with heart failure has good effect,and the indexes of cardiac function and the levels of IGF-1,D-dimer,cTnI and PCT are improved,with good safety,which can be popularized in clinic.
