上海小昆山社区高血压患者发生糖尿病的风险研究

Study of the risk of developing diabetes in hypertensive patients in Xiaokunshan Community

目的 :了解上海小昆山社区高血压患者患糖尿病风险。方法 :2014年至2018年,随机抽取上海小昆山社区3个居委会中未诊断糖尿病的1 402例高血压患者。其中男性657人(46.86%),平均年龄(65.96±8.46)岁;女性745人(53.14%),平均年龄(65.80±8.49)岁。采用回归模型分析糖尿病发病的影响因素。结果 :高血压队列的糖尿病发病率为28.32/千人年。Cox多因素回归模型显示,体质量指数和腰围增加、3级高血压、IGR史和血压控制是高血压患者发生糖尿病的影响因素。结论 :社区高血压患者的糖尿病发病风险较高。超重或肥胖、腹型肥胖、高血压病3级、IGR史是高血压患者发生糖尿病的危险因素,血压控制良好是保护因素。

Objective: To understand the risk of diabetes in hypertensive patients in Xiaokunshan Community, Shanghai.Methods: From 2014 to 2018, a total of 1 402 hypertensive patients without diabetes were randomly selected from the three neighborhood committees in Xiaokunshan Community, Shanghai. There were 657 males(46.86%), with an average age of(65.96± 8.46) years;there were 745 females(53.14%), with an average age of(65.80±8.49) years. Regression model was used to analyze the influencing factors of diabetes. Results: The incidence of diabetes in the hypertensive cohort was 28.32/thousand personyears. Cox multivariate regression model showed that increased body mass index and waist circumference, grade 3 hypertension,history of impaired glucose regulation(IGR) and blood pressure control were the influencing factors of diabetes in hypertensive patients. Conclusion: People with hypertension in the community are at higher risk of developing diabetes. Overweight or obesity, abdominal obesity, hypertension grade 3 and IGR history are risk factors for diabetes in patients with hypertension. Good blood pressure control is a protective factor.
