超声骨刀微创拔牙法治疗下颌水平阻生智齿患者的效果

Effects of ultrasound bone knife minimally invasive tooth extraction method in treatment of horizontal impacted mandibular wisdom teeth

目的:观察超声骨刀微创拔牙法治疗下颌水平阻生智齿患者的效果。方法:选取92例下颌水平阻生智齿患者为研究对象,按照随机数字表法分为观察组与对照组各46例。对照组行传统拔牙法治疗,观察组行超声骨刀微创拔牙法治疗,比较两组手术相关指标水平[手术时间、术中出血量、术后视觉模拟评分法(VAS)评分、术后肿胀程度和术后开口程度]、并发症发生率和炎性介质[穿透素-3(PTX3)、细胞间黏附分子-1(ICAM-1)、髓过氧化物酶(MPO)、前列腺素E_(2)(PGE_(2))]水平。结果:观察组手术时间短于对照组,术中出血量、术后VAS评分、术后肿胀程度均低于对照组,术后开口程度大于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率为8.70%,低于对照组的28.26%,差异有统计学意义(P<0.05);术后,观察组PTX3、ICAM-1、MPO、PGE_(2)水平均低于对照组,差异有统计学意义(P<0.05)。结论:超声骨刀微创拔牙法治疗下颌水平阻生智齿患者可改善手术相关指标水平,降低并发症发生率和炎性介质水平,效果优于传统拔牙法。

Objective:To observe effects of ultrasound bone knife minimally invasive tooth extraction method in treatment of horizontal impacted mandibular wisdom teeth.Methods:92 patients with horizontal impacted mandibular wisdom teeth were selected as the research objects and were divided into observation group and control group according to the random number table method,46 cases in each.The control group was treated with traditional tooth extraction method,while the observation group was treated with ultrasound bone knife minimally invasive tooth extraction method.The levels of surgery-related indicators[operation time,intraoperative blood loss,postoperative visual analogue scale(VAS)score,postoperative swelling degree and postoperative opening degree],the incidence of complications and the inflammatory mediator levels[pentraxin-3(PTX3),intercellular adhesion molecule-1(ICAM-1),myeloperoxidase(MPO),prostaglandin E_(2)(PGE_(2))]were compared between the two groups.Results:The operation time of the observation group was shorter than that of the control group;the intraoperative blood loss,the postoperative VAS score and the postoperative swelling degree were all lower than those of the control group;the postoperative opening degree was greater than that of the control group;and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 8.70%,which was lower than 28.26%in the control group,and the difference was statistically significant(P<0.05).After the surgery,the levels of PTX3,ICAM-1,MPO and PGE_(2) in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Ultrasound bone knife minimally invasive tooth extraction in the treatment of the patients with horizontal impacted mandibular wisdom teeth can improve the levels of surgery-related indicators,reduce the incidence of complications and the levels of inflammatory mediators.Moreover,it is superior to traditional tooth extraction method.
