心脏超声检查在高血压左心室肥厚伴左心衰竭诊断中的应用价值 被引量:2

摘要 目的:探讨心脏超声检查在高血压左心室肥厚伴左心衰竭诊断中的应用价值。方法:选取2018年2月至2021年2月该院收治的75例高血压左心室肥厚伴左心衰竭患者为研究组,另选取同期于该院行健康体检的75例志愿者为对照组。两组均行心脏超声检查,比较两组心脏超声参数[左心室质量指数(LVMI)、左室短轴缩短率(LVFS)、二尖瓣舒张早期血流峰速度/二尖瓣环舒张早期运动峰速度(E/Ea)及左室射血分数(LVEF)]、不同心功能分级患者心脏超声参数、心功能分级与LVMI、LVFS、E/Ea及LVEF的相关性。结果:研究组LVMI、LVFS、E/Ea均高于对照组,LVEF低于对照组,差异有统计学意义(P<0.05);研究组心功能Ⅲ级患者LVMI、LVFS、E/Ea均高于Ⅰ、Ⅱ级患者,且心功能Ⅱ级患者高于Ⅰ级患者;心功能Ⅲ级患者LVEF均低于Ⅰ、Ⅱ级患者,且心功能Ⅱ级患者低于Ⅰ级患者,差异统计学意义(P<0.05);经Pearson相关性分析显示,心功能分级与LVMI、LVFS、E/Ea呈正相关(P<0.05),与LVEF呈负相关(P<0.05)。结论:心脏超声检查应用于高血压左心室肥厚伴左心衰竭诊断中具有一定辅助价值,心功能分级与LVMI、LVFS、E/Ea呈正相关,与LVEF呈负相关,临床可根据超声参数变化判断患者病情。 Objective:To explore application value of echocardiogram in diagnosis of hypertensive left ventricular hypertrophy combined with left heart failure.Methods:75 patients with hypertensive left ventricular hypertrophy combined with left heart failure who were treated in this hospital from February 2018 to February 2021 were selected as study group,while 75 volunteers who underwent physical examinations in the hospital during the same period were selected as control group.Echocardiogram was performed in both groups.The echocardiographic parameter[left ventricular mass index(LVMI),left ventricular fractional shortening(LVFS),early diastolic peak mitral flow velocity/early diastolic peak mitral annular motion velocity(E/Ea)and left ventricular ejection fraction(LVEF)]were compared between the two groups.The echocardiographic parameter with cardiac function grading were also compared.The correlations among the cardiac function grading with the LVMI,LVFS,E/Ea and LVEF were analyzed.Results:The LVMI,LVFS,and E/Ea in the study group were higher than those in the control group;the LVEF was lower than that in the control group;and the differences were statistically significant(P<0.05).The LVMI,LVFS,and E/Ea in the patients with cardiac function grading III in the study group were higher than those in the patients with cardiac function grading I and II,and those in the patients with cardiac function grading II were higher than those in the patients with cardiac function grading I;the LVEF in the patients with cardiac function grading III were lower than those in the patients with cardiac function grading I and II,and those in the patients with cardiac function grading II were lower than those in the patients with cardiac function grading I;and the differences were statistically significant(P<0.05).The Pearson correlation analysis showed that the cardiac function grading was positively correlated with the LVMI,LVFS,and E/Ea(P<0.05),and negatively correlated with the LVEF(P<0.05).Conclusions:Echocardiogram has certain auxiliary value in the diagnosis of hypertensive left ventricular hypertrophy combined with left heart failure.The cardiac function grading is positively correlated with the LVMI,LVFS,E/Ea,and negatively correlated with the LVEF.Clinically,the patients’conditions can be judged according to the changes of echocardiogram parameters.
