舒适护理在老年慢性心力衰竭患者护理中的应用效果

Effect of Comfortable Nursing in Elderly Patients with Chronic Heart Failure

目的探讨舒适护理应用于老年慢性心力衰竭患者中的效果。方法选择本院2019年10月至2020年6月收治的慢性心力衰竭患者98例作为研究对象,并按照抽签法原则将所有患者均随机分为两组,即观察组(n=49)以及对照组(n=49)。对照组患者应用常规护理干预,观察组则在对照组基础上予以舒适护理干预,比较两组患者护理前后的生活质量、情绪变化情况,并使用舒适状况量表(GCQ)来评估护理后两组患者的舒适水平。结果护理前,两组患者的各方面舒适度评分相比,无明显差异(P>0.05),经过护理后,在心理、生理、环境及社会文化评分相对照,观察组患者的各方评分均明显高于对照组(P<0.05);经过护理后的数据统计,两组患者的HAMA以及HAMD评分均较护理干预前明显下降,且观察组低于对照组(P<0.05)。护理干预前,两组患者的血浆BNP水平、6 min步行距离对比,差异无统计学意义(P>0.05);经过护理干预后,观察组的血浆BNP水平、6 min步行距离显著优于对照组(P<0.05)。结论将舒适护理干预应用于老年慢性心力衰竭患者中有利于增强患者对抗疾病的自信,有效降低血浆BNP水平,改善6 min步行距离,有效缓解患者的负性情绪,提升患者的整体舒适度,并改善患者的生活质量。

Objective To explore the application effect of comfort care in elderly patients with chronic heart failure.Methods Ninety-eight patients with chronic heart failure admitted to our hospital from October 2019 to June 2020 were selected as research objects,and all patients were randomly divided into two groups according to the principle of drawing lots,namely observation group(n=49)and control group(n=49).Patients in the control group were given routine nursing intervention,while those in the observation group were given comfort nursing intervention on the basis of the control group.The quality of life and mood changes of patients in the two groups before and after nursing were compared,and the comfort level of patients in the two groups was evaluated by GCQ.Results Before nursing,there was no significant difference between the two groups in terms of comfort scores(P>0.05).After nursing,the scores of psychological,physiological,environmental and social culture in observation group were significantly higher than those in control group(P<0.05).According to the data statistics after nursing,HAMA and HAMD scores of the two groups were significantly lower than those before nursing intervention,and the observation group was lower than the control group(P<0.05).Before nursing intervention,there was no significant difference in plasma BNP level and 6-min walking distance between the two groups(P>0.05);after nursing intervention,the plasma BNP level and 6-min walking distance in the observation group were significantly better than those in the control group(P<0.05).Conclusion The application of comfort care interventions to elderly patients with chronic heart failure is beneficial to enhance the patients'confidence in fighting the disease,effectively reduces plasma BNP levels and improves the 6-min walking distance,which effectively helps patients relieve negative emotions and improve the overall comfort of patients,can improve the quality of life of patients to a large extent.
