构建“互联网+健康教育”社区糖尿病三级联动管理新模式

本文依托微信群、“巴渝健康”公众号、“微步有约”小程序,打造移动社区健康教育平台,构建“互联网+健康教育”社区糖尿病管理新模式。家庭医生以传播糖尿病健康知识与养成健康生活方式为目标,通过糖尿病训练营引导社区居民开展线上健康管理。糖尿病训练营提供了以“五驾马车”为核心的系列微视频和科普推文,鼓励参与者开展目标打卡,并在健康教育平台上记录血糖监测、饮食以及锻炼情况,以提升健康自我管理能力。从社区试点来看,参与训练营的综合满意度达80%以上。“互联网+健康教育”社区糖尿病管理新模式能促进健康生活方式养成,提升患者的自我管理能力。

This article relies on WeChat group,"Bayu health"official account,and the"We Walk"mini program to build a mobile community health education platform,and build a new mode of"Internet+Health education".Family doctors aim to disseminate diabetes health knowledge and develop healthy lifestyles,and guide community residents to carry out online health management through diabetes training camp.The diabetes training camp provides a series of micro videos and popular science articles with the core of"Five Chariots",encourages participants to launch daily attendance cards,and records blood glucose monitoring,diet and exercise on the platform of health education,so as to enhance the ability of self-management in health.From the perspective of community pilot,the comprehensive satisfaction of participating in the training camp reached more than 80%.The"Internet+Health education"community diabetes management mode can promote healthy lifestyle development and enhance patient self-management ability.
