基于数据挖掘国医大师张震治疗胃脘痛处方用药规律研究

Study on the Prescription and Medication Rules of Professor Zhang Zhen for Epigastric Pain Based on Data Mining

目的运用“名医传承平台”,探讨“国医大师”张震教授治疗慢性非萎缩性胃炎伴或不伴糜烂、慢性萎缩性胃炎所致胃脘痛的处方用药规律。方法筛选、录入田春洪教授2009年—2011年随诊张震教授记录的72份胃脘痛病例,将症状、处方用药等录入“名医传承平台”,并进行审核和核对;对核心及辅助药物、两味中药间关联度、中药与症状的关联度进行数据挖掘与分析;对于数据挖掘结果进行讨论,从而探讨张震教授治疗胃脘痛的处方用药规律。结果通过数据挖掘与分析,核心药物共有30味,排名前5位的依次为:甘草、柴胡、白芍、香附、木香;中药—中药关联度共得出5565个组合;对原始处方的伴随症状进行筛选,按出现频率排序,前4位依次为:胃脘胀痛、胃脘疼痛、失眠、胃脘胀闷。结论张震教授认为胃脘痛为中焦气机不利所致,治疗以疏肝、和胃、健脾为主,佐以消食化滞、活血化瘀、行气导滞、消痞散结等法。

Objective:To use the Famous Doctor Inheritance Platform to discuss the prescription and medication rules of Prof.Zhang Zhen,Master of Chinese Medicine,in the treatment of epigastric pain caused by chronic non-atrophic gastritis with or without erosion and chronic atrophic gastritis.Methods:72 epigastric pain patients recorded by Prof.Tian Chunhong who followed up Prof,Zhang Zhen from 2009 to 2011 were screened and entered,and then audited and checked.Data mining and analysis were carried out on the core and adjuvant drugs and the correlation between two-flavor Chinese medicine and the correlation between Chinese medicines and symptoms.The data mining results were discussed so as to explore the prescription and medication rules of Prof.Zhang Zhen for the treatment of epigastric pain.Results:Through the data mining and analysis,there were 30 core drugs,and the top 5 were Licorice,Bupleurum,Paeonia lactiflora,Cyperus rotundus and Costustoot.A total of 5565 combinations of traditional Chinese medicine and traditional Chinese medicine correlation was obtained.The accompanying symptoms of the prescription were screened and sorted according to the frequency of occurrence.The top 4 symptoms were epigastric swelling,epigastric pain,insomnia,and epigastric stuffying.Conclusion:Prof.Zhang Zhen believes that epigastric pain is caused by the unfavorable Qi mechanism of the middle burner Qi depression and the treatment is mainly to soothe the liver,harmonize the stomach and strengthen the spleen,supplemented by resolving food stagnation,promoting blood circulation to remove blood stasis,promoting the circulation of qi stagnation,and dissolving lumps and resolving masses.
