2019~2020年某中医院中药注射剂使用情况分析

Analysis on Usage of Traditional Chinese Medicine Injections in a Traditional Chinese Medicine Hospital from 2019 to 2020

目的:了解重庆市永川区中医院中药注射剂的使用情况,为临床合理用药提供参考。方法:从医院信息系统及国家药品不良反应监测系统中统计2019~2020年本院中药注射剂的销售金额、用药频度(DDDs)、限定日费用(DDC)、药品销售金额排序(B)/DDDs排序(A)的比值、不良反应(ADR)等相关数据并进行分析。结果:2019~2020年中药注射剂的销售金额略有上升,祛瘀剂、清热剂和肿瘤用药居前3位,祛瘀剂使用最广泛,品种数占比超过50%,销售金额占比超过60%,且DDDs排名前10位的基本都为祛瘀剂;不论是销售金额还是DDDs,注射用血塞通和注射用血栓通均居前2位;DDC排名前10位的中药注射剂的DDDs均不大,其中康艾注射液和蟾酥注射液的B/A<0.5,且有下降趋势;不良反应发生率较2015~2018年有所下降。结论:本院中药注射剂的使用基本合理,但对部分品种仍需加强监控,防止中药注射剂滥用。

Objective:To understand the use of traditional Chinese medicine injections in the Yongchuan hospital of traditional Chinese medicine,and to provide references for clinic rational drug use.Methods:From the hospital information system and the National Adverse Drug Reaction Monitoring System,the consumption sum,defined daily dose system(DDDs),defined daily cost(DDC),ratio of sequence of consumption sum(B)/sequence of DDDs(A),adverse reaction(ADR)and other relevant data were counted and analyzed.Results:From 2019 to 2020,the consumption sum of traditional Chinese medicine injections increased slightly,and stasis-removing agents,heat-clearing agents and tumor drugs ranked first 3,and the most widely used was stasis-removing agents,the number of varieties accounted for more than 50%,and the consumption sum accounted for more than 60%,and the top 10 ranked by DDDs are basically stasisremoving agents.Xuesaitong injection and Xueshuantong injection both rank in the top 2 regardless of the consumption sum or DDDs.The DDDs of top 10 traditional Chinese medicine injections ranked by DDC are not large.Among them,the B/A value of Kangai injection and Chansu injection is less than 0.5,and there is a downward trend;the incidence of adverse reactions has decreased compared with that in 2015~2018.Conclusion:The use of traditional Chinese medicine injections in the Yongchuan Hospital of traditional Chinese medicine is basically reasonable,but it is still necessary to strengthen monitoring of some varieties to prevent abuse of TCM injections.
