临床药师在延续护理中的作用

The Role of Clinical Pharmacists in Transitional Care

近年来,患者在延续护理期间的药物安全问题在不同国家和地区都得到了大量关注。我国医疗机构的患者再入院率较高可能与延续护理制度不完善、患者不完全了解复杂药物治疗方案而错误用药导致的严重药物不良事件(ADE)等有关。本文通过阐述临床药师在改善延续护理方面的作用和责任,总结国内外临床药师在延续护理期间面临的挑战,参考国外临床药师参与下的部分护理模式,提出了针对我国临床药师参与改善延续护理的系统性建议,以期为相关政策和制度的建立提供参考。

In recent years,the drug safety of patients during transitional care has received a lot of attention in different countries and regions.The high readmission rate of patients in medical institutions in our country may be related to serious adverse drug events(ADE)caused by incomplete transitional care,patients’incomplete understanding of complex drug treatment regimens and misuse of drugs.By expounding the roles and responsibilities of clinical pharmacists in improving transitional care,this paper summarizes the challenges faced by clinical pharmacists at home and abroad during transitional care.Referring to some nursing models with the participation of clinical pharmacists in foreign countries,this paper proposes a method for improving transitional care with the participation of clinical pharmacists in our country.Systematic recommendations,with a view to providing reference for the establishment of relevant policies and systems.
