浅谈感染科门诊住培医生临床思维培养

Experience on Outpatient Clinical Thinking Training of Resident-training Doctors in Infectious Diseases Department

住院医师规范化培训阶段是医学生走出学校大门成长为一名合格的医生的必经阶段,对保证临床医师专业水准和医疗服务质量具有极为重要的作用。感染科教学门诊是将教学与感染科门诊相结合,以学生为主导,教师从旁指导的一种临床教学模式。门诊教学过程中,学生可直接接触真实的病例,充分发挥其在学习中的主观能动性,强化其临床思维能力训练,使其达到对疾病的深刻认识。感染科门诊教学是综合医院住院医师临床培养过程中非常重要的环节,对其临床思维和决策能力培养有显著意义。笔者结合住院医师规范化培养模式及个人多年来临床住院医师带教的经验,从医学基础知识的掌握、完整病史的采集、详细的体格检查、实验室检查资料的收集、人文关怀的重要性、严密的逻辑思考等方面探讨了在感染科门诊住培医生临床思维培养的相关体会,并从临床角度整理了几点感染科门诊住院医师规范化培训中的问题,并尝试提出一些解决的方法。

Resident standardized training stage is a necessary stage for medical students to grow up to be a qualified doctor,which plays an important role in ensuring the professional level of clinicians and the quality of medical services.The teaching out-patient department of infectious diseases is a clinical teaching mode that combines teaching with the outpatient department of infectious diseases,with students as the leading factor and teachers as the guidance.In the process of outpatient teaching,students can directly contact with real cases,give full play to their subjective initiative in learning,and strengthen the training of clinical thinking ability,so as to achieve a profound understanding of the disease.The outpatient teaching of infectious diseases department is a very important link in the process of clinical training of residents in general hospitals,which is of great significance to the cultivation of their clinical thinking and decision-making ability.Based on the standardized training mode of residents and personal experience in clinical resident teaching for many years,the author discussed the relevant experience of clinical thinking training of resident doctors in infectious diseases department from the aspects of mastering basic medical knowledge,collecting complete medical history,detailed physical examination,collecting laboratory examination data,the importance of humanistic care,and strict logical thinking From the clinical point of view,this paper sorted out some problems in the standardized training of outpatient residents in infectious diseases department,and tried to put forward some solutions.
