超重与肌性血管动脉僵硬度的关系研究

摘要 目的初步探讨超重与肌性血管高动脉僵硬度的关系及影响因素。方法随机选取2418例受试者,依据年龄由高至低五分位数分为≥69岁(Q1组)、≥61.9岁(Q2组)、≥51.6岁(Q3组)、≥38岁(Q4组)和<38岁(Q5组)。获取受试者一般资料后,采用袖带振荡波获取受试者肌性动脉僵硬度指标动脉-容积指数(API)。以API≥31定义为高API,以体质量指数(BMI)≥24 kg/m^(2)诊断为超重,行分层分析。比较五组内超重和对照组受试者的API和高API发生率的差异。采用Pearson相关和多因素Logistic回归模型分别分析高API发生率的危险因素。结果①API值和收缩压在五组间差异有统计学意义,且随年龄增加而增加(均P<0.01)。②在Q1、Q2、Q4及Q5组内,超重者API值高于对照组受试者(均P<0.05)。③单因素相关分析提示,API与年龄、BMI和收缩压呈正相关(r=0.504、0.138和0.699,均P<0.01),与身高和脉搏呈负相关(r=-0.071和-0.116,均P<0.01)。采用Logistic回归模型校正身高、年龄、收缩压、舒张压和脉搏的影响,超重者发生高API风险是非超重者1.403倍,优势比OR(95%CI)=1.403(1.111,1.695)(P<0.05);男女受试者发生高API风险相似,OR(95%CI)=1.295(0.883,1.707)(P>0.05)。结论超重者与体质量正常者API和高API均存在差异,体质量越大API值越大。超重者发生高动脉僵硬度的风险更高。 Objective To discuss the relationship between overweight and high muscular arterial stiffness and risk factors.Methods 2418 subjects were randomly selected and divided into groups of≥69 years old(Q1),≥61.9 years old(Q2),≥51.6 years old(Q3),≥38 years old(Q4),and<38 years old(Q5),according to age range from high to low quintile.After obtaining general information,cuff oscillation wave was used to obtain arterial-volume index(API,muscular arterial stiffness index).API≥31 was defined as high API,and body mass index(BMI)≥24 kg/m^(2)was diagnosed as overweight for stratified analysis.Differences in API and high API incidence between overweight and control subjects in 5 groups were compared.Pearson correlation and multivariate Logistic regression models were used to analyze risk factors for high API incidence.Results API and systolic blood pressure had statistically significant differences between 5,and they increased with age(allP<0.01).In Q1,Q2,Q4,and Q5 groups,API of overweight subjects was higher than that of control subjects(P<0.05).Univariate correlation analysis showed API was positively correlated with age,body mass index,and systolic blood pressure,respectively(r=0.504,0.138,and 0.699,allP<0.01),and negatively correlated with height and pulse(r=-0.071 and-0.116,allP<0.01).Effects of height,age,systolic blood pressure,diastolic blood pressure,and pulse were adjusted by logistic regression.Overweight subjects were 1.403 times more likely to have a high API than non-overweight people.Odds ratioOR(95%CI)=1.403(1.111,1.695)(P<0.05).Male and female subjects had similar risks of high API,OR(95%CI)=1.295(0.883,1.707)(P>0.05).Conclusion There are differences in API and high API between overweight people and normal-weight subjects.Heavier weight would have a greater API.Subjects who are overweight would have a higher risk of developing high arterial stiffness.
