中药离子导入联合神阙穴隔药灸治疗寒凝血瘀型原发性痛经临床观察

Clinical Observation on Chinese Medicine Iontophoresis Combined with Shenque(CV8) Medicine-separated Moxibustion in the Treatment of Primary Dysmenorrhea of Cold Congealing and Blood Stasis Type

目的 观察中药离子导入联合神阙穴隔药灸治疗寒凝血瘀型原发性痛经的临床效果。方法 选取2020年5月—2020年7月吉州区妇幼保健计划生育服务中心和井冈山大学附属医院妇产科就诊的50例原发性痛经患者,采用随机数字表法分为2组,治疗组(中药离子导入联合神阙穴隔药灸)24例和对照组(常规处理)23例,2组均以1个月经周期为1个疗程,共3个疗程,对比2组患者治疗后的临床疗效,中医证候积分,视觉疼痛模拟评分,采用ELISA测定2组患者PGF2α、IL-6、TNF-α的表达水平。结果 疗程结束后,治疗组总有效率高于对照组;治疗组VAS评分和中医证候积分明显低于对照组(P<0.05);对照组在3个月后随访VAS评分和中医证候积分明显高于治疗组(P<0.05);治疗组PGF2α、IL-6、TNF-α含量水平明显低于对照组(P<0.05)。结论 中药离子导入联合神阙穴隔药灸能明显改善寒凝血瘀型原发性痛经患者的症状,可能与调节PGF2α、IL-6、TNF-α的表达水平相关。

Objective To observe the clinical effect of iontophoresis combined with Shenque(CV8)medicine-separated moxibustion in the treatment of primary dysmenorrhea of cold congealing and blood stasis type.Methods 50 Patients with primary dysmenorrhea treated in Obstetrics and Gynecology department of Jinggangshan University Affiliated Hospital from May 2020 to July 2020 were divided into 2 groups by random number table method,24 cases in the treatment group(traditional Chinese medicine ion introduction combined with moxibustion to the umbilicus) and 23 cases in the control group(conventional treatment).Each group took one menstrual cycle as a course of treatment,a total of 3 courses.The clinical efficacy,traditional Chinese medicine(TCM) syndrome score and visual pain simulation score were compared between 2 groups after treatment.The expression levels of PGF2 α,IL-6 and TNF-α in 2 groups were determined by ELISA.Results After the course of treatment,the total effective rate of the treatment group was higher than that of the control group,and the VAS scores and TCM syndrome scores of the treatment group were significantly lower than those of the control group(P<0.05).After 3 months’ follow-up,the VAS scores and TCM syndrome scores in the control group were significantly higher than those in the treatment group(P<0.05),and the levels of PGF2,IL-6 and TNF in the treatment group were significantly lower than those in the control group(P<0.05).Conclusion Iontophoresis combined with Shenque(CV8)medicine-separated moxibustion can obviously improve the symptoms of primary dysmenorrhea of cold congealing blood stasis type,which may be related to regulating the expression level of PGF2,IL-6 and TNF.
