多系统萎缩的MRI特征:与帕金森病对比

The MRI features of multiple system atrophy:comparison with those of Parkinson’s disease

摘要 目的探讨磁共振成像(magnetic resonance imaging,MRI)诊断多系统萎缩(multiple system atrophy,MSA)影像学特征。方法收集经临床证实的MSA、帕金森病(Parkinson’s disease,PD)患者各50例,以同期本院门诊体检的50例健康人员作为对照组,比较小脑中脚、脑桥十字征T2W1高信号征发生率、测量并比较小脑中脚宽度、中脑面积、脑桥面积,对比分析三组壳核、脑桥、额叶白质ADC值,总结诊断MSA、PD的影像学特征。结果MSA组小脑中脚、脑桥十字征T2W1高信号发生率明显高于PD组及对照组,差异有统计学意义(P<0.05)。MSA组小脑中脚宽度、中脑面积、脑桥面积明显小于PD及正常组(P<0.05);PD组及对照组小脑中脚宽度、中脑面积、脑桥面积差异无统计学意义(P>0.05)。MSA组壳核、桥脑、额叶白质ADC均高于PD组及正常组(P<0.05);PD组壳核、桥脑、额叶白质ADC与对照组相比,差异无统计学意义(P>0.05)。结论磁共振成像在诊断多系统萎缩方面表现出一定特征性,其定量参数能为临床提供一定影像依据。 Objective To explore the imaging features of magnetic resonance imaging(MRI)in the diagnosis of multiple system atrophy(MSA).Methods This study collected 50 cases of MSA and 50 cases of Parkinson’s disease(PD)confirmed by clinical diagnosis.Meanwhile,50 healthy subjects who underwent physical examination in our hospital during the same period were taken as the control group.The researchers compared their incidence of high-intensity signal on T2W1 images for middle cerebellar peduncle and hot-cross bun sign,and also measured and compared their middle cerebellar peduncle width,midbrain area,and the pons area.The research compared and analyzed the ADC values of putamen,pons,and frontal white matter of three groups,and then summarized the imaging features of MSA and PD diagnosed by MRI.Results The incidence of high-intensity signal on T2W1 images for middle cerebellar peduncle and hot-cross bun sign in the MSA group was higher than that of the PD group and that of the control group,indicating statistically significant difference(P<0.05).The middle cerebellar peduncle width,midbrain area,and the pons area of MSA group were significantly smaller than those of the PD group and of the control group(P<0.05).There was no statistically significant difference in middle cerebellar peduncle width,midbrain area,and the pons area between the PD group and the control group(P>0.05).The ADC values of putamen,pons and frontal white matter of MSA group were higher than those of the PD group and those of the control group(P<0.05).There was no statistically significant difference in the ADC values of putamen,pons and frontal white matter between PD group and the control group(P>0.05).Conclusion MRI shows certain characteristics in the diagnosis of MSA,and its quantitative parameters can provide a certain image basis for clinical practice.
