重症急性胰腺炎患者预后的影响因素分析

Analysis of factors affecting the prognosis of patients with severe acute pancreatitis

目的探讨重症急性胰腺炎(SAP)患者预后的影响因素,以评估SAP患者预后风险,并采取有效干预措施改善患者预后。方法回顾性分析本院2018年1月至2020年1月收治的90例SAP患者的临床资料,统计患者的基线资料,包括性别、年龄、天冬氨酸转氨酶(AST)、总蛋白(TP)、白蛋白(Alb)、血尿素氮(BUN)、血肌酐(SCr)水平、是否大量饮酒、是否高脂饮食、是否胆囊泥沙样结石、是否高脂血症。统计患者预后情况,分析其预后影响因素。结果90例SAP患者中,病死12例,病死率为13.33%。经单因素、多项Logistic回归分析显示,大量饮酒、高脂饮食、胆囊泥沙样结石、高脂血症均为影响SAP患者预后不良的危险因素(OR>1,P<0.05)。结论大量饮酒、高脂饮食、胆囊泥沙样结石、高脂血症会增加SAP患者病死风险,未来可据此评估SAP患者的预后,对降低患者病死率具有积极意义。

Objective To investigate the factors affecting the prognosis of patients with severe acute pancreatitis(SAP),so as to assess the prognosis risk of SAP patients,and to take effective intervention measures to improve the prognosis of patients.Methods The clinical data of 90 SAP patients admitted to our hospital from January 2018 to January 2020 were retrospectively analyzed,and the baseline data of the patients were counted,including gender,age,aspartate aminotransferase(AST),total protein(TP),Albumin(Alb),blood urea nitrogen(BUN),blood creatinine(SCr)level,whether heavy drinking,whether high-fat diet,whether gallbladder silt-like stones,whether hyperlipidemia.Statistics on the prognosis of patients and analysis of factors affecting their prognosis.Results Of the 90 SAP patients,12 patients died,and the fatality rate was 13.33%.Single factor and multiple Logistic regression analysis showed that heavy drinking,high-fat diet,gallbladder silt-like stones,and hyperlipidemia were the risk factors for prognosis of SAP patients(OR>1,P<0.05).Conclusion Heavy drinking,high-fat diet,gallbladder silt-like stones,and hyperlipidemia can increase the risk of death in SAP patients.In the future,the prognosis of SAP patients can be evaluated based on this,and it is of positive significance to reduce the death rate of patients.
