慢性阻塞性肺疾病患者脑小血管病MRI影像学特征的检出情况

Detection of MRI imaging characteristics of cerebral small vessel disease in chronic obstructive pulmonary disease patients

目的:探讨脑小血管病(CSVD)MRI影像学特征在慢性阻塞性肺疾病(COPD)患者中的检出情况。方法:纳入2019年3月至2021年3月郑州大学第五附属医院收治的因头晕、头昏并有慢性支气管炎或肺气肿病史就诊,年龄≥45岁的患者115例,均行脑部MRI检查,经肺功能检查确诊COPD 59例,余56例为非COPD组。记录患者的CSVD MRI影像学特征(高级别脑白质高信号、腔隙性梗死、脑微出血、高级别血管周围间隙扩大)检出情况。结果:COPD组吸烟患者比例大于非COPD组(P<0.05),两组其他一般资料差异无统计学意义。COPD组高级别脑白质高信号检出率大于非COPD组(25.4%vs 8.9%,P<0.05);两组其他CSVD MRI影像学特征比较,差异无统计学意义。结论:COPD与CSVD有关联。

Aim:To investigate detection of MRI imaging characteristics of cerebral small vessel disease(CSVD)in chronic obstructive pulmonary disease(COPD)patients.Methods:A total of 115 patients older than 45 years with chief complaints of dizziness and history of chronic bronchitis or emphysema treated in the Fifth Affiliated Hospital of Zhengzhou University from March 2019 to March 2021 were enrolled.All the patients had brain MRI.Among them,59 patients were diagnosed as COPD by lung function assessment.MRI characteristics of CSVD were recorded,including high-grade white matter hypersignal,lacunar infarction,cerebral microbleed,and high-grade enlarged perivascular space.Results:The proportion of smoking patients in COPD group was higher than that in non-COPD group(P<0.05).The detection rate of high signal intensity of white matter in COPD group was higher than that in non-COPD group(25.4%vs 8.9%,P<0.05).There were no significant differences between the 2 groups in the other imaging characteristics.Conclusion:COPD is associated with CSVD.
