胰头部实性假乳头状瘤的临床特点和外科治疗

Clinical features and surgical treatment of solid pseudopapillary tumor of the pancreatic head

目的总结胰头部胰腺实性假乳头状瘤(SPTP)的临床特点和外科治疗方法.方法回顾性分析中国科学技术大学附属第一医院(安徽省立医院)胆胰外科从2006年1月至2021年1月诊治的29例胰头部SPT临床资料.结果本组29例患者中男4例、女25例,男女比例为1∶6.25;平均年龄为29.3(11~39)岁.大多数患者首发症状表现为上腹部疼痛,血清学胆红素及肿瘤标记物均在正常范围内.所有患者均接受手术治疗,其中12例患者行胰头部肿块切除术,11例行根治性胰十二指肠切除术,6例行全腹腔镜胰十二指肠切除术.所有患者术后病理均证实为胰腺实性假乳头状瘤.术后随访率为86.2%.随访时间2~180个月,无死亡病例.结论胰头部SPTP患者临床罕见,多见于年轻女性,术前鉴别诊断困难,手术完整切除为最佳治疗方案,总体预后好.

Objective To summarize the clinical features and surgical treatment of solid-pseudopapillary tumor(SPTP)of pancreatic head.Methods The clinical data of 13 cases of solid-pseudopapillary tumor of pancreatic head in the Affiliated Anhui Pro vincial Hospital from January 2006 to January 2021 were retrospectively analysized.Results Of the 29 patients,4 males and 25 females,M/F ratio was 1:6.25;mean age was 29.3 years,with a range from 11 to 39,Most patients first symptoms of upper abdominal pain,serum bilirubin and tumor markers were within the normal range.All patients were underwent surgicial treatment,including tumor enucleation in 12 cases,pancreaticoduodenectomy in 11 cases,laparoscopic pancreaticoduodenectomy in 6 cases,all patients were pathologically confirmed solid-pseudopapillary tumor.Postoperative follow-up rate was 86.2%.Up from 2 to 180 months,there were no deaths.Conclusions Solid-pseudopapillary tumor of pancreatic head are clinically rare which is difficult to diagnose preoper-atively,occurring mainly in young women.Complete surgical resection is the best treatment,overall prognosis is good.
