3.0T MRI多b值弥散加权技术在前列腺良恶性结节中的鉴别价值 被引量:2

摘要 目的:探究3.0T MRI多b值弥散加权成像(diffusion weighted imaging,DWI)技术在前列腺良恶性结节中的鉴别诊断价值。方法:对2018年11月—2020年11月佛山市第一人民医院收治的68例男性前列腺病变患者资料回顾性分析,依据病理结果将患者分为良性组(n=38)与恶性组(n=30),所有患者均接受常规MRI、3.0T MRI多b值DWI检查,以病理活检结果为金标准,分析两种检查方式的灵敏度、特异度及准确率,并比较两组患者的表观弥散系数(apparent diffusion coefficient,ADC)值的差异。结果:3.0T MRI多b值DWI检查灵敏度、特异度和准确率分别96.67%(29/30)、94.74%(36/38)和95.59%(65/68),均显著高于常规检查技术的80.00%(24/30)、84.21%(32/38)和82.35%(56/68)(P<0.05)。不同b值良性结节组ADC值均显著高于恶性组(P<0.05)。结论:3.0T MRI多b值DWI技术有助于前列腺恶性及良性结节的鉴别诊断,具有较高临床价值。 Objective To explore the differential diagnosis value of 3.0T MRI multi-b-value diffusion weighted imaging(DWI)in benign and malignant prostate nodules.Methods The data of 68 male patients with prostate lesions treated in Foshan first People's Hospital from November 2018 to November 2020 were retrospectively analyzed.According to the pathological results,the patients were divided into benign group(n=38)and malignant group(n=30).All patients received routine MRI and 3.0T MRI multi-bvalue DWI examinations,and pathological biopsy results were taken as the gold standard.The sensitivity,specificity and accuracy of the two methods were analyzed,and the differences in apparent diffusion coefficient(ADC)values between the two groups were compared.Results The sensitivity,specificity and accuracy of conventional MRI were 80.00%(24/30),84.21%(32/38)and 82.35%(56/68),respectively.The sensitivity,specificity and accuracy of 3.0T MRI multi-b value DWI were 96.67%(29/30),94.74%(36/38)and 95.59%(65/68),respectively,which were significantly higher than those of routine examination techniques80.00%(24/30),84.21%(32/38)and 82.35%(56/68)(P<0.05).The ADC values of benign nodules were significantly higher than those of malignant nodules(P<0.05).Conclusion 3.0T MRI multi-b value DWI technique is helpful in differential diagnosis of malignant and benign prostatic nodules.
