住院患者病情严重程度分级评估工具的构建研究

Construction of a grading assessment tool for inpatients’severity of disease

目的构建适用于住院患者的病情严重程度分级评估工具,为科学有效评估患者病情及合理划分护理级别提供依据。方法选取2020年9月至11月在黑龙江省某三级医院住院患者共300例,采用疾病严重程度评价量表和简单临床评分评估患者的病情严重程度,并依据经典测量理论中的相关系数法、因子分析法和克朗巴赫系数法进行指标的筛选重组,进而构建住院患者病情严重程度分级评估工具,并进行信效度验证。结果病情分级评估工具经过条目筛选共保留了12项指标,经过验证,其Cronbach’sα系数为0.660,分半信度系数为0.686,探索性因子分析共提取4个因子,累计贡献率为62.541%。结论本研究构建的住院患者病情严重程度分级评估工具具有良好的信效度,有助于我国护理分级制度的发展和完善,形成科学、量化的护理分级标准,合理配置医疗资源。

Objective To construct a grading assessment tool for inpatient severity of disease,scientifically and effectively evaluate the patients’illness,and reasonably divide the nursing level.Methods A cross-sectional survey was used to select inpatients from 10 departments of medicine and surgery in a tertiary hospital in Heilongjiang Province from September 2020 to November 2020.The disease severity assessment scale and simple clinical score were used to evaluate the severity of the patient’s disease,and the indicators were screened and reassembled according to the correlation coefficient method factor analysis method and cronbach coefficient method in classical measurement theory,so as to build the inpatient severity of disease grading assessment tool and verify its reliability and validity.Results A total of 12 indicators were retained by the disease grading assessment tool after item screening,after verification,the was 0.660,the fractional and half reliability coefficient was 0.686,and a total of 4 factors were extracted by exploratory factor analysis,with a cumulative contribution rate of 62.541%.Conclusions The assessment tool for the severity of diseasegrading of inpatients constructed in this study has a good reliability and validity,which is conducive to the development and improvement of the nursing grading system in China,the formation of a scientific and quantitative nursing grading standard,and the rational allocation of medical resources.
