肺结核与支原体肺炎患者外周血淋巴细胞亚群及细胞因子分析

Analysis of lymphocyte subsets and cytokines in peripheral blood of patients with pulmonary tuberculosis and mycoplasma pneumonia

目的通过淋巴细胞亚群和细胞因子检测,比较肺结核和支原体肺炎患者淋巴细胞亚群和细胞因子的差别。方法选择我院结核病防治所确诊肺结核患者60例,支原体肺炎患者30例,健康查体30例为对照组,分别检测血液中CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),CD_(19)^(+),CD_(16)^(+),CD_(56)^(+)淋巴细胞亚群的绝对数、百分数及血浆中IL-2,IL-4,IL-6,IL-10,TNF-α,IFN-γ细胞因子水平,统计组间各指标P值。结果两组间CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),CD_(19)^(+)数值比较差异有统计学意义(P<0.05);除CD_(8)^(+)在支原体肺炎组与对照组、CD_(19)^(+)在肺结核组与支原体肺炎组外,其余组间两两比较差异均有统计学意义(P<0.05);两组间淋巴细胞亚群百分比差异无统计学意义(P>0.05);组间TNF-α,IFN-γ,IL-4差异有统计学意义(P<0.05),除IL-4的肺结核组与支原体肺炎组外,其余组间两两比较,差异均有统计学意义(P<0.05)。结论肺结核和支原体肺炎患者外周血淋巴细胞CD_(8)^(+)和CD_(19)^(+)水平相似,肺结核患者CD_(3)^(+),CD_(4)^(+),CD_(8)^(+)较低,细胞因子TNF-α和IL-4较高,IFN-γ较低。

Objective To compare the differences of lymphocyte subsets and cytokines in patients with tuberculosis and mycoplasma pneumonia through the detection of lymphocyte subsets and cytokines.Methods It selected 60 confirmed pulmonary tuberculosis patients,30 mycoplasma pneumonia patients,and 30 healthy patients as the control group.The absolute numbers and percentage of CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),CD_(19)^(+),CD_(16)^(+),and CD_(56)^(+) lymphocyte subsets in blood and the levels of IL-2,IL-4,IL-6,IL-10,TNF-α,and IFN-γ cytokines in plasma were detected,and the P value of each index between the groups was calculated.Results There were significant differences in absolute numbers of CD_(3)^(+),CD_(4)^(+),CD_(8)^(+) and CD_(19)^(+) between the two groups(P<0.05),except for CD_(8)^(+) between the mycoplasma pneumonia group and the control group,and CD_(19)^(+) between the tuberculosis group and the mycoplasma pneumonia group(P<0.05).There was no significant difference in the percentage of lymphocyte subsets between the two groups(P>0.05).The differences in TNF-α,IFN-γ and IL-4 among the groups were statistically significant(P<0.05),except for the level of IL-4 between the tuberculosis group and the mycoplasma pneumonia group,the differences between the other groups were statistically significant(P<0.05).Conclusion The levels of CD_(8)^(+) and CD_(19)^(+) in peripheral blood lymphocytes in patients with tuberculosis and mycoplasma pneumonia are similar,and the levels of CD_(3)^(+),CD_(4)^(+) and CD_(8)^(+) in patients with tuberculosis are lower,and cytokines TNF-α and IL-4 are higher,and IFN-γ is lower.
