菌群移植成功诱导缓解重度溃疡性结肠炎1例并文献复习

摘要 老年激素依赖型重度复发性溃疡性结肠炎的诱导缓解是临床治疗的难点。目前,指南推荐应用生物制剂、免疫抑制剂及手术治疗,本例老年患者拒绝接受外科手术,生物制剂、免疫抑制剂都有明确的副作用,因此探讨无创有效诱导缓解的新思路、新方法是研究的热点。本例患者采用粪菌进行配型成功后,给予菌群移植-口服粪菌胶囊,连续服用1个月后,病情得到缓解,口服粪菌胶囊成功诱导缓解。2019年美国胃肠病协会提出,菌群移植可能可以有效诱导缓解轻中度溃疡性结肠炎患者的临床症状,但是本例老年患者是重度复发性溃疡性结肠炎,仍然得到成功诱导缓解,可见该疗法值得临床推广和借鉴。 Old hormone-dependent severe recurrent ulcerative colitis is the difficulty of treatment in induced remission.At present,the guidelines recommend the use of biological agents,immunosuppressants and surgical treatment.This elderly patient refused to accept surgery,and biological agents and immunosuppressants have clear side effects.It is a research hotspot to explore new ideas and methods of noninvasive and effective induction of remission.After the successful matching of fecal bacteria,this patient is given fecal microbiota transplantation-oral fecal bacteria capsule,after continuous administration for 1 month,the condition of the patients is alleviated,and oral fecal bacteria capsule successfully induced remission.In 2019,the American Gastroenterology Association proposed that fecal microbiota transplantation may effectively induce and alleviate the clinical symptoms of patients with mild to moderate ulcerative colitis.This elderly patient is a severe recurrent ulcerative colitis,but he has still been induced remission successfully.It suggests that this therapy is worthy of clinical promotion and reference.
