玻璃体腔内注射雷珠单抗、全视网膜光凝联合AGV植入术治疗新生血管性青光眼的临床效果

Clinical effect of intravitreal injection of ranibizumab,whole retinal photocoagulation and AGV implantation in the treatment of neovascular glaucoma

目的探讨玻璃体腔内注射雷珠单抗、全视网膜光凝联合Ahmed青光眼引流阀(AGV)植入术治疗新生血管性青光眼的临床效果。方法回顾性选择2017年6月至2020年6月本院收治的76例新生血管性青光眼患者作为研究对象,根据治疗方法不同将其分为对照组(36例)和研究组(40例)。对照组采用全视网膜光凝联合AGV植入术治疗,研究组在对照组基础上采用玻璃体腔内注射雷珠单抗治疗。比较两组的治疗效果。结果研究组的治疗总有效率高于对照组(P<0.05)。手术1个月后,两组的眼压低于手术前1 d,视力评分高于手术前1 d,且研究组优于对照组(P<0.05)。手术1个月后,两组的PSD、MD值均优于手术前1 d,且研究组优于对照组(P<0.05)。研究组的并发症总发生率低于对照组(P<0.05)。结论玻璃体腔内注射雷珠单抗、全视网膜光凝联合AGV植入术治疗新生血管性青光眼的效果显著,可改善患者眼压、眼部视野及视力水平。

Objective To investigate the clinical effect of intravitreal injection of ranibizumab,whole retinal photocoagulation and Ahmed glaucoma valve(AGV)implantation in the treatment of neovascular glaucoma.Methods A total of 76 patients with neovascular glaucoma treated in our hospital from June 2017 to June 2020 were retrospectively selected as the research objects.According to different treatment methods,the patients were divided into control group(36 cases)and study group(40 cases).The control group was treated with whole retinal photocoagulation combined with AGV implantation,and the study group was treated with intravitreal injection of ranibizumab on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment of the study group was higher than that of the control group(P<0.05).One month after operation,the intraocular pressure of the two groups was lower than that one day before operation,the visual acuity score was higher than that one day before operation,and those of the study group were better than the control group(P<0.05).One month after operation,the PSD and MD value in the two groups were better than those one day before operation,and those in the study group were better than the control group(P<0.05).The total incidence of complications in the study group was lower than that in the control group(P<0.05).Conclusion Intravitreal injection of ranibizumab,whole retinal photocoagulation and AGV implantation in the treatment of neovascular glaucoma has a significant effect,which can improve the intraocular pressure,ocular visual field and visual acuity level.
