头针运动疗法治疗急性期腰椎间盘突出症的随机对照研究

Scalp Acupuncture Therapy for Acute Lumbar Disc Herniation:A Randomized Controlled Study

目的比较头针运动疗法与体针治疗急性期腰椎间盘突出症的疗效差异。方法60例患者随机分为头针组与体针组各30例。头针组采用针刺头部特定条带进行治疗,选取顶中带、顶枕带中1/3、顶颞前斜带上1/5(单侧病变取病变对侧,两侧病变取双侧)、顶颞后斜带上1/5(单侧病变取病变对侧,两侧病变取双侧)、顶枕带下。体针组采用针刺肾俞(双)、大肠俞(双)、委中(患侧)、承山(患侧)治疗。于治疗前后评估两组视觉模拟量表(VAS)、Osuestry功能障碍指数量表(ODI)的评分差异。结果头针组总有效率为93.33%,高于体针组的83.33%(P<0.01);两组治疗第2周、第4周后VAS与ODI评分均较治疗前下降(P<0.01),组间比较头针组评分明显优于体针组(P<0.01)。结论头针运动疗法治疗急性期腰椎间盘突出症可减轻患者疼痛,改善腰腿部功能障碍,提高患者生活质量,疗效优于常规体针治疗。

Objective:To compare the effect of scalp acupuncture and body acupuncture for acute lumbar disc herniation(LDH).Methods:A total of 60 patients were randomly divided into a scalp acupuncture group and a body acupuncture group,with 30 cases in each group.The scalp acupuncture group was treated with acupuncture specific strips on the head,selecting the parietal middle zone,the middle 1/3 of the parieto-occipital zone,the upper 1/5 of the parietal anterior oblique zone(the contralateral side of the lesion for unilateral lesions,and the bilateral lesions for both sides),and the upper 1/5 of the parietotemporal posterior oblique zone(choosing the opposite side of the lesion for thesingle side lesions,and both sides for the lesions on both sides),under the parieto-occipital belt.The body acupuncture group was treated with acupuncture on Shenshu(double sides),Dachangshu(double sides),Weizhong(affected side),and Chengshan(affected side).The difference in the scores of VAS and ODI between the two groups before and after treatment was evaluated.Results:The total effective rate of scalp acupuncture group was 93.33%,and the total effective rate of body acupuncture group was 83.33%(P<0.01);the VAS and ODI scores of both groups after the 2nd and 4th week of treatment were decreased compared with those before treatment(P<0.01).and the scores scalp acupuncture group were significantly better than the body acupuncture group(P<0.01).Conclusion:Scalp acupuncture therapy in the treatment of acute lumbar disc herniation can relieve pain,improve waist and leg dysfunction,and improve the quality of life of patients.The curative effect is better than conventional body acupuncture.
