早期护理干预对维持性血透患者动静脉内瘘并发症发生率的影响

Effect of early nursing intervention on the incidence of arteriovenous fistula complications in maintenance hemodialysis patients

目的 研究将早期护理干预应用在维持性血液透析患者当中对患者动静脉内瘘并发症发生率的影响。方法 本文行分组对照试验,选择2018年1月至2019年8月于本院进行维持性血液透析动静脉内瘘的76例患者,分别将早期护理和常规护理干预方法应用在两组患者的护理中并比较效果。结果 (1)观察组(4例,占10.53%)相比对照组(15例,占39.47%),并发症发生率的整体水平明显更低(P<0.05)。(2)观察组(35例,占92.11%)相比对照组(28例,占73.68%),具有更高的满意度水平(P<0.05)。(3)护理之前,两组患者的GAD-7、PHQ-9、PSQI评分之间均没有显著性差异(P>0.05),护理后,两组患者的GAD-7、PHQ-9、PSQI评分有明显的降低,但观察组的降低水平比对照组更高,满足统计学差异性(P<0.05)。(4)护理之前,两组患者的SF-36生活质量量表评分之间均没有显著性差异(P>0.05),护理后,两组患者的SF-36生活质量量表评分有明显的升高,而且观察组的升高水平比对照组更高(P<0.05)。结论 针对维持性血液透析动静脉内瘘的患者使用早期护理可有效减少并发症出现的概率,使患者对护理更加满意。这项护理可以转变患者的负性心理和睡眠障碍的情况,提高患者的整体生活质量,有推荐的价值。

Objective To study the efiect of early nursing intervention in maintenance hemodialysis patients on the incidence of arteriovenous fistula complications. Methods A grouped controlled trial was conducted in this study, and 76 patients with arteriovenous fistulas undergoing maintenance hemodialysis in our hospital from January 2018 to August 2019 were selected. and compare the efiects. Results(1) Compared with the control group(15 cases, 39.47%) in the observation group(4 cases, accounting for 10.53%), the overall level of the incidence of complications was significantly lower(P<0.05).(2) The observation group(35 cases, accounting for 92.11%) had a higher level of satisfaction than the control group(28 cases, accounting for 73.68%)(P<0.05).(3) Before nursing, there was no significant difierence in GAD-7, PHQ-9 and PSQI scores between the two groups of patients(P>0.05). After nursing, the GAD-7, PHQ-9 and PSQI scores of the two groups of patients were significantly reduced,but the reduction level of the observation group was higher than that of the control group(P<0.05).(4) Before nursing, there was no significant difierence in the scores of SF-36 quality of life scale between the two groups of patients(P>0.05), after nursing, the SF-36 quality of life scale scores of the patients in the two groups were significantly increased, but the elevated level in the observation group was higher than that in the control group(P<0.05). Conclusion Trying early nursing for patients with maintenance hemodialysis arteriovenous fistula can efiectively reduce the incidence of complications and make patients more satisfied with nursing. This care can transform the patient’s negative psychology and sleep disorders, and improve the patient’s overall quality of life, which is worth recommending.
