儿童替牙期OPG与RANKL表达水平分析

摘要 目的分析儿童替牙期破骨细胞抑制因子(OPG)和核因子κB受体活化因子配体(RANKL)表达水平,旨在探究OPG和RANKL在下颌恒中切牙的萌出过程中是否起调节作用。方法随机抽取南昌市市区10所幼儿园和小学的5~6岁儿童共424名进行口腔检查。将受试者按照年龄和下颌中切牙牙冠萌出程度分成4组,其中A组和B组分别为5岁、6岁儿童解剖牙冠萌出1/2组,C组和D组分别为5岁、6岁儿童解剖牙冠全萌出组,每组106例。用吸潮纸尖提取被检儿童的龈沟液,利用ELISA法检测其OPG、RANKL的表达水平。结果A组与C组、B组与D组比较,其OPG及RANKL表达水平差异均无统计学意义(P>0.05),但二者表达水平随着下颌中切牙的萌出而降低。结论不同萌出程度的下颌恒中切牙的龈沟液中均存在OPG和RANKL,且其含量存在个体差异;同年龄组解剖冠全萌时OPG与RANKL的平均浓度较解剖冠萌出1/2时均有所下降,提示二者在牙萌出过程中可能有一定的调节作用。 Objective To investigate the regulating role of osteoprotegerin(OPG)and receptor activator of nuclear factor-κB ligand(RANKL)in the process of mandibular permanent central incisor eruption by analyzing their expression in children during dental replacement.Methods Oral examination was performed in 424 children aged 5-6 years who were randomly selected from 10 kindergartens and primary schools in Nanchang.According to the age and the degree of mandibular central incisor eruption,these subjects were divided into 4 groups:group A(5-year-old children with 1/2 eruption of tooth crown),group B(6-year-old children with 1/2 eruption of tooth crown),group C(5-year-old children with complete eruption of tooth crown),and group D(6-year-old children with complete eruption of tooth crown).The gingival crevicular fluid was collected with absorbent paper,and the expression of OPG and RANKL was detected by ELISA.ResultsComparison between group A and group C,group B and group D,the expression levels of OPG and RANKL were not significantly different(P>0.05).However,OPG and RANK expression decreased with the eruption of mandibular central incisors.Conclusion Both OPG and RANKL are present in gingival crevicular fluid of mandibular permanent central incisors with different degrees of eruption,and their contents are different between individuals.In the same age group,the average concentrations of OPG and RANKL in children with complete eruption of tooth crown are lower than those in children with 1/2 eruption of tooth crown,indicating that OPG and RANKL may play a regulatory role in the process of tooth eruption.
