学龄前支原体肺炎患儿病情严重程度与白介素-4、白介素-6、白介素-8、肿瘤坏死因子-α水平相关性分析

Correlation between severity of mycoplasma pneumonia and levels of IL-4,IL-6,IL-8 and TNF-αin preschool children with mycoplasma pneumonia

目的分析支原体肺炎患儿病情严重程度与白介素-4(IL-4)、白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平相关性。方法选取2020年1月至2021年6月郑州大学附属儿童医院接收的94例支原体肺炎学龄前患儿进行分析。根据肺炎严重指数(PSI)程度评分为普通支原体肺炎58例(普通组),重症支原体肺炎36例(重症组)。另选取94名健康体检的学龄前儿童作为对照组。检验三组儿童血清IL-4、IL-6、IL-8、TNF-α水平。对学龄前支原体肺炎患儿采用PSI评估病情严重程度,分析IL-4、IL-6、IL-8、TNF-α水平与学龄前支原体肺炎患儿病情严重程度的相关性及诊断价值。结果普通组儿童PSI评分(78.45±9.57)分低于重症组儿童的(103.84±9.75)分(P<0.05)。三组儿童血清IL-4、IL-6、IL-8、TNF-α水平比较差异有统计学意义(P<0.05)。IL-4、IL-6、IL-8、TNF-α水平与支原体肺炎患儿PSI评分均呈正相关(P<0.05)。受试者工作特征曲线(ROC)曲线分析可知,IL-4的曲线下面积(AUC)值为0.753,IL-6的AUC值为0.736,IL-8的AUC值为0.747,TNF-α的AUC值为0.711(P<0.05)。结论IL-4、IL-6、IL-8、TNF-α水平与支原体肺炎患儿病情严重程度具有相关性,可用于学龄前支原体肺炎患儿病情的辅助诊断。

【Objective】To analyze the correlation between the severity of mycoplasma pneumonia and the levels of interleukin-4(IL-4),interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α).【Methods】A total of 94 preschool children with mycoplasma pneumonia received in our hospital from January 2020 to June 2021 were selected.According to the severity index of pneumonia,58 cases of common mycoplasma pneumonia(common group)and 36 cases of severe mycoplasma pneumonia(serious group)were scored.In addition,94 preschool children who came to our hospital for physical examination were selected as the control group.The levels of serum IL-4,IL-6,IL-8,and TNF-αin the three groups of children were tested.Pneumonia severity index(PSI)was used to evaluate the severity of the disease in preschool children with mycoplasma pneumonia.The correlation between the levels of IL-4,IL-6,IL-8,TNF-αand the severity of preschool children with mycoplasma pneumonia and the diagnostic value of these indexes was analyzed.【Results】The average PSI score of children in the general group was 78.45±9.57 points,and the average PSI score of the severe group children was 103.84±9.75 points;there was a significant difference between the groups(P<0.05).The differences in serum levels of IL-4,IL-6,IL-8 and TNF-αamong the three groups of children were statistically significant(P<0.05).The levels of IL-4,IL-6,IL-8 and TNF-αwere positively correlated with the PSI scores of children with mycoplasma pneumonia(P<0.05).ROC curve analysis showed that the AUC value of IL-4 was 0.753,the AUC value of IL-6 was 0.736,the AUC value of IL-8 was 0.747,and the AUC value of TNF-αwas 0.711(P<0.05).【Conclusion】The levels of IL-4,IL-6,IL-8,and TNF-αare correlated with the severity of mycoplasma pneumonia in children,and can be used for auxiliary diagnosis of mycoplasma pneumonia in preschool children.
