摘要
目的探讨慢性心力衰竭(CHF)和肥胖(体质量指数≥30 kg/m2)人群外周血白细胞及其分类和比值的差异。方法采用美国国家健康与营养调查(NHANES)2007~2016年的公开数据资料,比较CHF和肥胖人群的外周血白细胞及其分类计数和中性粒细胞与淋巴细胞的比值(NLR)的差异。结果与对照组相比,CHF组患者外周血白细胞总数、粒细胞及单核细胞计数和NLR比值均高于对照组,但淋巴细胞计数低于对照组(P均<0.05);在非CHF人群中,肥胖者外周血白细胞总数、粒细胞、单核细胞及淋巴细胞计数均高于非肥胖者(P均<0.05),但NLR无明显差异(P>0.05);CHF伴肥胖组患者除血中性粒细胞稍增高外(P=0.046),外周血白细胞及其他分类计数和NLR比值较非肥胖CHF组患者无差异(P均>0.05)。结论CHF和肥胖人群均存在外周血免疫状态的改变,但CHF合并肥胖患者外周血白细胞计数和NLR比值与非肥胖的CHF患者无明显差异。
Objective To explore the difference of peripheral blood leukocytes and their classsification and ratio bewteen people with chronic heart failure(CHF)and obesity(body mass index,BMI≥30 kg/m2).Methods Public data from the National Health and Nutrition Examination Survey(NHANES)from 2007 to 2016 was used to compare the peripheral blood leukocytes and their classification counts and neutrophils to lymphocytes ratio(NLR)between CHF and obese participants.Results The total number of leukocytes,granulocytes and monocytes and NLR in peripheral blood were higher while the lymphocyte count was lower significantly in patients with CHF than those in the control group(all P<0.05).In non-CHF group,the total number of leukocytes,granulocytes,monocytes and lymphocytes in peripheral blood of obese group were higher than those of non-obese group(all P<0.05)while there was no signficant difference in NLR(P>0.05).However,there was no significant difference in peripheral blood leukocytes and their classification counts and NLR between obese CHF patients and non-obese CHF patients,except that blood neutrophils increased slightly.Conclusions Changes in peripheral blood immune status were observed in both CHF patients and obese population,but there were no significant differences in blood leukocyte counts and NLR between CHF patients with and without obesity.
