NKD2基因甲基化与口腔鳞癌临床特征和预后的相关性

摘要 目的探讨NKD2基因甲基化与口腔鳞癌临床特征和预后的相关性。方法选择178例口腔鳞癌患者作为研究对象,提取所有患者的外周静脉血,检测NKD2基因甲基化情况,并调查所有患者的临床资料并随访预后。结果在178例患者中,NKD2基因高甲基化78例,占43.8%;高甲基化组的吸烟史、糖尿病史、Ⅲ级+Ⅳ级病理分级等比例显著高于低甲基化组(P<0.05)。随访到2019年7月,高甲基化组的平均生存时间为(20.32±2.18)个月,显著短于低甲基化组的(27.48±1.78)个月(t=9.742,P=0.000)。在178例患者中,直线相关分析NKD2基因高甲基化与肿瘤病理分级、生存时间、吸烟史、糖尿病史显著相关(P<0.05)。多因素Logistic回归分析显示:NKD2基因高甲基化、吸烟史、糖尿病史、病理分级为影响患者预后的主要因素(P<0.05)。结论口腔鳞癌NKD2基因甲基化比较常见,NKD2基因高甲基化可影响患者的临床特征与预后,有可能成为口腔鳞癌新的生物标志物。 Objective To investigate the relationship between methylation of NKD2 gene and clinical features and prognosis in oral squamous cell carcinoma.Methods 178 patients with oral squamous cell carcinoma were selected as subjects.The peripheral venous blood of all patients were extracted and the NKD2 gene methylation were detected,the clinical status and the prognosis of all patients were investigated.Results There were 78 patients were hypermethylated with NKD2 gene that accountedg for 43.8%.The smoking history,diabetes history,gradeⅢ+Ⅳpathological grade in the hypermethylation group were significantly higher than those in the hypomethylated group(P<0.05).The mean survival time of the hypermethylated group were 20.32±2.18 months,which were significantly lower than that of the hypomethylated group,27.48±1.78 months(t=9.742,P=0.000).Linear correlation analysis of NKD2 gene hypermethylation were significantly associated with pathological grade,survival time,smoking history,and diabetes history(P<0.05).Multivariate logistic regression analysis showed that NKD2 gene hypermethylation,smoking history,diabetes history,and pathological grade were the main factors affecting the prognosis of patients(P<0.05).Conclusion The methylation of NKD2 gene in oral squamous cell carcinoma is common.The hypermethylation of NKD2 gene may affect the clinical features and prognosis of patients,and may become a new biomarker for oral squamous cell carcinoma.
