基于古代文献的血瘀证针灸选穴思路探析

Exploration on the rules of acupoint selection for Blood stasis syndrome with acupuncture and moxibustion based on ancient literature

目的:分析古代医籍针灸治疗血瘀证的取穴规律,探究血瘀证针灸治疗原则,指导现代临床治疗。方法:通过关键词搜索《中华医典》针灸推拿类医籍,根据纳入排除标准筛选条文,建立数据库。对选用腧穴进行频次统计及关联、聚类分析,探究其选穴规律,对古代医家针灸治疗血瘀证的思路进行总结。结果:治疗血瘀证频次较高的穴位有气海、足三里、天枢等;使用频次较高的特定穴为交会穴、募穴、合穴;常用经脉是任脉、足太阳膀胱经和足阳明胃经;关联性最大的一组配穴为中极-气海;聚类分析将所选腧穴分为3类。结论:古代医家针灸治疗血瘀证取穴注重气血同治和调补脾胃,主调气补土以行血。

Objective: To analyze the rule of thought of acupuncture and moxibustion for treating blood stasis syndrome, and to explore its treatment principles of blood stasis syndrome so as to provide guidance to clinical treatment. Methods: Key words was taken to search the Zhonghuayidian(Electronic dictionary of Chinese medicine), the relevant items were filtered by inclusion and exclusion criteria and establish a database. The frequency statistics, correlation analysis and cluster analysis of acupoints were carried out, and the rules of acupoint selection were explored, and the ideas of acupuncture and moxibustion in ancient medical scholar were summarized. Results: It was found out that the highest frequently used acupoints were Qihai, Zusanli, Tianshu;the highest frequently used specific acupoints were the crossing point, Mu-front point and He-sea point. The highest frequently used meridian were the Conception vessel, the Urinary Bladder Meridian of Foot-Taiyang, the Stomach Meridian of Foot-Yangming. ZhongjiQihai couple with the most correlation. The selected acupoints were divided into three categories by cluster analysis. Conclusion: Ancient medical scholar laid emphasis on adjusting Qi and the Spleen Meridian and Stomach Meridian. They proposal to smooth the movement of Qi so as to breed the Spleen Meridian and Stomach Meridian, in this way to activate blood circulation.
