基于上臂袖带振荡波评估动脉僵硬度的新指标及其影响因素 被引量:1

摘要 目的 应用上臂袖带振荡波技术获取动脉僵硬度新指标,并探讨其影响因素。方法 招募2020年8~12月上海交通大学附属第一人民医院健康体检的志愿者2 581例,依据年龄五分位数,即把年龄由大到小排列并分为五等分,分为≥69岁组651例、≥61.9岁组484例、≥51.6岁组482例、≥38岁组464例和<38岁组500例。采用袖带振荡波分析法获取所有受试者动脉波速指数(AVI)。其中,124例受试者采集了左心室射血分数(EF)。采用Pearson相关和线性回归分析分别分析AVI和年龄、身高、体质量、体质量指数及EF的相关性。结果 五组受试者的AVI值比较差异均有显著统计学意义(P<0.01),且AVI随年龄增加而增加,差异均有显著统计学意义(P<0.01);受试者的AVI与年龄呈线性相关(P<0.05),AVI与年龄、体质量指数、收缩压和舒张压呈正相关(r=0.539、0.072、0.486、0.192,P<0.01),与EF和身高呈负相关(r=-0.246、-0.111,P<0.01),但与体质量无相关性(r=-0.034,P=0.096)。结论 AVI是反映动脉僵硬度的新指标,其与年龄、体质量指数及左室射血分数密切相关。 Objective To obtain a new index of arterial stiffness by a cuff oscillometirc waveform analysis(COW) and to explore its influencing factors. Methods A total of 2 581 volunteers undergoing physical examination in Shanghai General Hospital Affiliated to Shanghai Jiao Tong University from August 2020 to December 2020 were recruited and divided into 5 groups according to age [≥69 years old group(651 cases), ≥61.9 years old group(484 cases), ≥51.6 years old group(482 cases), ≥38 years old group(464 cases), and <38 years old group(500 cases)]. Arterial velocity pulse index(AVI) was obtained by COW to analyze arterial stiffness. Moreover, the left ventricular ejection fraction(EF) of 124 subjects was obtained by echocardiography. Pearson correlation and linear regression analysis were used to analyze the correlation between AVI and age, height, weight, body mass index, and EF, respectively. Results There were significant differences in AVI values among the five groups(all P<0.01), and AVI increased with age(all P<0.01).In the five groups, there was a linear correlation between AVI and age(all P<0.05). In the total subjects, AVI was positively correlated with age, body mass index, systolic and diastolic blood pressure, respectively(r=0.539, 0.072, 0.486and 0.192, all P<0.01), negatively correlated with EF and height(r=-0.246,-0.111, P<0.01), and had no significant correlation with weight(r=-0.034, P=0.096). Conclusion Arterial velocity pulse index is a new index reflecting arterial stiffness, which is closely related to age, body mass index, and left ventricular EF.
