摘要 目的探讨大面积脑梗死患者给予依达拉奉联合奥拉西坦治疗的临床效果。方法采用随机抽样法选取2018年7月至2019年7月本院收治的90例大面积脑梗死患者。通过随机数表法分组:对照组(45例)给予依达拉奉治疗,研究组给予依达拉奉联合奥拉西坦治疗。比较两组的疗效。结果两组治疗前美国国立卫生院卒中量表(NIHSS)评分、血清肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)水平、血清神经肽Y(NPY)、粒细胞巨噬细胞-集落刺激因子(GM-CSF)水平比较,差异无统计学意义(P>0.05);治疗后,研究组NIHSS评分低于对照组(P<0.05),研究组TNF-α、CRP、NPY、GM-CSF水平低于对照组(P<0.05)。研究组总有效率高于对照组(P<0.05)。结论在大面积脑梗死患者治疗中联用依达拉奉和奥拉西坦可减轻神经功能缺损,增强疗效,考虑作用机制与降低NPY、GM-CSF表达、控制炎性反应有关。 Objective To study the clinical effect of edaravone combined with oxiracetam in patients with large area cerebral infarction.Methods From July 2018 to July 2019,90 patients with large area cerebral infarction were selected from our hospital.The randomized digital table method was divided into two groups:control group(45 cases)and edaravone treatment,and the study group received a combination of edaravone and olara.The effect of the two groups was compared.Results The National Institutes of Health Stroke Scale(NIHSS)score and serum tumor necrosis factor before treatment-α(TNF-α),C-reactive protein(CRP)and serum neuropeptide Y(NPY)and granulocyte macrophage colony stimulating factor(GM-CSF)levels were no significant difference(P>0.05).After treatment,the NIHSS score in the study group was lower than that in the control group(P<0.05);the study group serum TNF-α,CRP,NPY and GM-CSF in the control group were lower than those in the control group(P<0.05).The total effective rate of the study group was higher than that of the control group(P<0.05).Conclusion The combination of edaravone and olaxitan in the treatment of patients with large-area cerebral infarction can reduce the neurological deficit and enhance the curative effect.The mechanism is related to reducing the expression of serum NPY and GM-CSF and controlling the inflammatory response.
