巨细胞病毒DNA和葡萄糖-6-磷酸脱氢酶对新生儿高胆红素血症的诊断意义 被引量:1

摘要 目的分析巨细胞病毒(CMV)-DNA和葡萄糖-6-磷酸脱氢酶(G6PD)水平对新生儿高胆红素血症的诊断意义。方法选取该院高胆红素血症新生儿作为观察组(n=196),另选取同期新生儿作为对照组(n=100),分别采用FQ-PCR技术与生化法检测两组母乳、新生儿尿液CMV-DNA和血清G6PD水平。绘制ROC曲线,比较CMV-DNA、G6PD单独与联合检测的敏感度和特异度。结果观察组母乳、新生儿尿液的CMV阳性率和血清G6PD缺乏率均高于对照组(P<0.05)。ROC结果显示,观察组新生儿中CMV-DNA、G6PD单独及两者联合检测的敏感度分别为51.7%、60.8%、66.9%,特异度分别为69.4%、80.2%、89.7%。结论高胆红素血症新生儿CMV-DNA及G6PD阳性检出率均高,CMV-DNA和G6PD联合检测对其诊断具有临床指导意义。 Objective To analyze the significance of cytomegalovirus(CMV)DNA and glucose-6-phosphate dehydrogenase(G6PD)level in the diagnosis of neonatal hyperbilirubinemia.Methods The neonates with hyperbilirubinemia in the hospital were selected as the Observation Group(n=196),and healthy neonates at the same period were selected as the Control Group(n=100).FQ-PCR and biochemical methods were used to detect the CMV-DNA and serum G6PD level in breast milk and neonatal urine of the two groups respectively.The ROC curve was drawn and the sensitivity and specificity of G6PD,CMV and their combination in the detection were compared.Results The CMV positive rates and serum G6PD deficiency in breast milk and neonatal urine of the Observation Group were higher than those in the Control Group(P<0.05).ROC results showed that the sensitivity of G6PD,CMV and their combination in the Observation Group was 51.7%,60.8%,and 66.9%,respectively,and the specificity was 69.4%,80.2%,and 89.7%,respectively.Conclusion The neonates with hyperbilirubinemia have a high detection rate of CMV-DNA and G6PD positive,and the combined detection of CMV-DNA and G6PD is of clinical guiding significance for the diagnosis of hyperbilirubinemia.
