2015—2019年英德市居民恶性肿瘤疾病负担分析

Study on the burden of malignant tumors among residents in Yingde City from 2015 to 2019

目的分析2015—2019年英德市居民恶性肿瘤的疾病负担,为有针对性地制定肿瘤防治措施提供依据。方法通过国家死因登记信息系统,收集2015—2019年英德市恶性肿瘤患者死亡数据,用死亡率和伤残调整寿命年(DALY)估算英德市恶性肿瘤的疾病负担和间接经济负担。结果2015—2019年英德市居民恶性肿瘤年平均死亡率为133.19/10万,男性为172.58/10万,大于女性的91.00/10万(χ^(2)=722.981,P<0.05)。2015—2019年英德市恶性肿瘤总DALY损失为107079.58人年,DALY率为每千人口18.40人年,男性DALY率大于女性,差异有统计学意义(wilcoxon W=15.0,P=0.009)。恶性肿瘤疾病负担主要集中在≥50岁的居民,疾病负担居前五位的分别是支气管癌或肺癌、肝癌、胃癌、结直肠癌、口腔和口咽癌,不同性别恶性肿瘤的疾病负担顺位不同。2015—2019年英德市恶性肿瘤所造成的间接经济负担为151165.64万元。结论英德市恶性肿瘤疾病较高,应重点防控≥50岁男性人群。支气管癌或肺癌、肝癌、胃癌是危害英德市居民健康的主要肿瘤。

Objective To analyze the disease burden of malignant tumors among residents in Yingde City from 2015 to2019,so as to formulate targeted tumor prevention and treatment measures.Methods Through the National Death Cause Registration Information System,the death data of patients with malignant tumors in Yingde City from 2015 to 2019 were collected,and the mortality and disability-adjusted life year(DALY)were used to estimate the disease burden and indirect economic burden of malignant tumors in Yingde City.Results From 2015 to 2019,the average annual mortality rate of malignant tumors among residents in Yingde City was 133.19/100000,and that of men was 172.58/100000,which was higher than 91.00/100000 compared with women(χ^(2)=722.981,P<0.05).The total DALY loss of malignant tumors in Yingde City from 2015 to 2019 was 107079.58 person-years,and the DALY rate was 18.40 per thousand people.The rate of DALY of females was higher than males(wilcoxon W=15.0,P=0.009).The intensity of malignant tumor loss was mainly concentrated in≥50 years of age.The top 5 disease burdens were bronchial or lung cancer,liver cancer,gastric cancer,colorectal cancer,oral cavity and oropharyngeal cancer.The order of the disease burden of malignant tumors was different for men and women.The indirect economic burden caused by malignant tumors in Yingde City from 2015 to 2019 was 1511.6564 million yuan.Conclusion The disease burden of malignant tumors in Yingde City is higher.The focus should be on the prevention and control of men over 50 years old.Bronchial or lung cancer liver cancer,and gastric cancer are key tumors that endanger the health of residents in Yingde City.
