胰岛素强化治疗对早期2型糖尿病肾病患者的影响

Observe the Effect of Intensive Insulin Therapy on Early Type 2 Diabetic Nephropathy

目的探讨在早期2型糖尿病肾病的治疗中应用胰岛素强化治疗的临床效果。方法选取2019年1月至2021年1月盘锦市人民医院收治的86例早期2型糖尿病肾病患者作为研究对象,按随机数字表法分为对照组与观察组,各43例。单纯接受口服降糖药物治疗的为对照组,在口服降糖药物治疗基础上联合胰岛素强化治疗的为观察组,比较两组的治疗效果。结果观察组治疗有效率为95.35%,高于对照组的76.74%,差异有统计学意义(P<0.05);治疗后,观察组空腹血糖(FBG)、餐后2 h血糖(2hPBG)、糖化血红蛋白(HbAlc)低于对照组,差异有统计学意义(P<0.05);治疗后,观察组尿微量白蛋白排泄率(UAER)、β_(2)-微球蛋白(β;-MG)、胆固醇(TC)、三酰甘油(TG)低于对照组,差异有统计学意义(P<0.05)。结论早期2型糖尿病肾病患者接受胰岛素强化治疗在稳定血糖、提高治疗效果、改善蛋白尿等方面效果显著,能有效延缓糖尿病肾病进程,与口服降糖药物相比优势明显。

Objective To analyze the effects of intensive insulin therapy in the treatment of early type 2 diabetic nephropathy. Methods A total of 86 patients with early type 2 diabetic nephropathy admitted to Panjin City People’s Hospital from January 2019 to January 2021 were randomly divided into groups, with 43 cases in each group. Those who received oral hypoglycemic drugs alone served as the control group, and were combined with insulin on the basis of oral hypoglycemic drugs. The intensive treatment was the analysis group, and the treatment effects and blood glucose and other indicators of the 2 groups were compared. Results The total effective rate of early type 2 diabetic nephropathy in the analysis group was 95.35%, which was statistically significant compared with 76.74% in the control group(P<0.05). After treatment, the analysis group and the control group showed FBG, 2 h PG, The HbAlc and UAER values were lower, and the difference was statistically significant(P<0.05);after treatment, the analysis group of patients β_(2)-MG, TC, and TG were all lower than the control group, the difference was statistically significant(P<0.05). Conclusion Intensive insulin therapy for patients with early type 2 diabetic nephropathy has significant effects in stabilizing blood sugar, improving therapeutic effects,and improving proteinuria. It can effectively delay the process of diabetic nephropathy and has more significant advantages compared with oral hypoglycemic drugs.
