高频超声联合应变式弹性成像评估腹直肌分离患者腹直肌间距和腹白线弹性

An evaluation of inter-rectus distance and linea alba elasticity using high frequency ultrasound combined with strain elastography in patients with diastasis recti abdominis

目的 采用高频超声联合应变式弹性成像评估腹直肌分离(DRA)患者腹直肌间距(IRD)和腹白线弹性。方法 采用高频超声联合应变式弹性成像评估产后1年内DRA女性患者101例(观察组)和未生育女性健康体检者88例(对照组)3个状态(静息、屈曲和呼气末)下3个位点(剑突与脐中点、脐水平和脐与耻骨联合中点)的IRD和腹白线弹性。分析观察组腹白线弹性评分与IRD相关性。结果 两组3种状态下脐与耻骨联合中点IRD均为零。观察组3种状态下剑突与脐中点和脐水平IRD均大于对照组,而腹白线弹性评分均低于对照组(P<0.05)。观察组3种状态下剑突与脐中点、脐水平的腹白线弹性评分与IRD均呈负相关(P<0.05)。结论 高频超声联合应变式弹性成像可以较好评估DRA患者IRD和腹白线弹性。

Objective To evaluate the inter-rectus distance(IRD) and linea alba elasticity in the patients with diastasis recti abdominis(DRA) through high frequency ultrasound combined with strain elastography.Methods High frequency ultrasound combined with strain elastography was used to evaluate the IRD and linea alba elasticity in 101 female patients with DRA(group A) within 1 year after delivery and 88 healthy non-pregnant women(group B),which was performed at 3 sites of midpoint of xiphoid and umbilicus, umbilicus level and midpoint of umbilicus and pubic symphysis in 3 states of at rest, flexion and end-expiration.The correlation between elastic score of linea alba and IRD in group A was analyzed.Results The IRD of midpoint of umbilicus and pubic symphysis in 3 states was 0 in both groups.The IRD of midpoint of xiphoid and umbilicus and umbilicus level in group A was larger than that in group B,while the elastic score of linea alba in group A was lower than that in group B(P<0.05).In group A,elastic scores of linea alba in 3 states were negatively correlated with IRD at midpoint of xiphoid and umbilicus and umbilicus level in group A(P<0.05).Conclusion High frequency ultrasound combined with strain elastography can better evaluate IRD and elasticity of linea alba in the patients with DRA.
