任务导向性双侧上肢训练对脑卒中早期患者日常生活能力的影响 被引量:4

Effect of task-oriented bilateral upper limbs training on daily living ability in patients with stroke in early stage

摘要 目的 观察任务导向性双侧上肢训练对脑卒中早期患者日常生活能力的影响。方法50例脑卒中偏瘫患者随机分成试验组和对照组,每组25例。所有患者接受常规康复干预。试验组接受以任务导向性训练为基础设计的双侧上肢运动训练;对照组接受重复节律性双侧上肢运动训练。运动训练每次30 min,每天1次,每周5 d,共4周。分别于干预前后采用Fugl-Meyer上肢运动功能评定(FMA-UE)、改良Barthel指数(MBI)和患侧握力及生存质量测定量表简表(QOL-BREF)对疗效进行评价。结果 干预前,两组患者FMA-UE评分、MBI评分和患侧握力及QOL-BREF评分比较差异均无统计学意义(P>0.05)。干预后,两组各项指标均较干预前改善(P<0.01),且试验组MBI评分及QOL-BREF评分较对照组改善更明显(P<0.05)。结论 对脑卒中早期患者采用任务导向性双侧上肢训练,可以更好地改善日常生活能力及生存质量。 Objective To observe the effect of task-oriented bilateral upper limbs training on the ability of daily living in the patients with stroke in early stage.Methods Fifty stroke patients with hemiplegia were randomly divided into two groups with 25 cases each.All patients received routine rehabilitation intervention.The group A received bilateral upper limbs training based on task-oriented training and group B received repetitive rhythmic training of both upper limbs, which were performed 30 minutes once a day for four weeks.Fugl-meyer assessment of upper-extremity section(FMA-UE),modified Barthel index(MBI),grip strength of the affected side and quality of life brief scale(QOL-BREF) were compared before and after intervention.Results The FMA-UE score, MBI score, the grip strength of the affected side and QOL-BREF score were not significantly different before and after intervention(P>0.05),which were improved after intervention in both groups(P<0.01).The scores of MBI and QOL-BREF after intervention were better in group A than those in group B(P<0.05).Conclusion Task-oriented bilateral upper limb training can better improve the ability of daily living and quality of life in the patients with stroke in early stage.
