叙事护理对食管癌患者情绪及癌因性疲乏的影响

Effect of narrative nursing on emotion and cancer-related fatigue of patients with esophageal cancer

目的探讨叙事护理对食管癌患者情绪及癌因性疲乏的影响。方法将78例食管癌患者按照随机数字表法分为研究组与对照组,每组39例。两组均给予常规护理,研究组在此基础上给予叙事护理,观察3周。干预前后采用汉密顿焦虑量表及汉密顿抑郁量表评定两组焦虑抑郁状况,采用简易应对方式问卷评定两组应对方式,采用癌症疲乏量表评定两组疲乏程度,采用癌症治疗功能评价量表评定两组生活质量。结果干预后两组汉密顿焦虑量表、汉密顿抑郁量表及癌症疲乏量表各维度评分均较干预前显著降低(P<0.01),研究组显著低于对照组(P<0.01)。干预后两组简易应对方式问卷积极应对维度评分较干预前显著升高(P<0.01),消极应对维度评分较干预前显著降低(P<0.01),研究组较对照组变化更显著(P<0.01)。干预后研究组癌症治疗功能评价量表各维度评分及总分均较干预前显著升高(P<0.01),且显著高于对照组(P<0.05或0.01)。结论叙事护理能缓解食管癌患者的焦虑抑郁情绪,改善应对方式,减轻癌因性疲乏程度,提高患者的生活质量。

Objective To explore the effect of narrative nursing on emotion and cancer-related fatigue of patients with esophageal cancer.Methods A total of 78 patients with esophageal cancer were divided into study group and control group according to random number table method,with 39 cases in each group.Both groups were given routine nursing,and the study group was given narrative nursing on this basis,and the observation was carried out for 3 weeks.Before and after the intervention,the anxiety and depression status of the two groups were evaluated by Hamilton anxiety scale(HAMA) and Hamilton depression scale(HAMD),the coping styles of the two groups were evaluated by simplified coping style questionnaire(SCSQ),the fatigue degree of the two groups was evaluated by cancer fatigue scale(CFS),and the quality of life of the two groups was evaluated by functional assessment of cancer therapy-generic scale(FACT-G).Results After the intervention,the scores of HAMA,HAMD and CFS in the two groups were significantly lower than those before the intervention(P<0.01),and the study group was significantly lower than the control group(P<0.01).After the intervention,the positive coping subscale scores of SCSQ in the two groups were significantly higher than those before the intervention(P<0.01),and the negative coping subscale scores were significantly lower than those before the intervention(P<0.01),the study group changed more significantly than the control group(P < 0.01).After the intervention,the FACT-G scores and total scores of the study group were significantly higher than those before the intervention(P<0.01),and were significantly higher than those of the control group(P<0.05 or 0.01).Conclusions Narrative nursing can relieve the anxiety and depression of patients with esophageal cancer,improve coping style,reduce cancer-related fatigue,and improve the quality of life of patients.
