颈动脉支架成形术中血管痉挛与术后再狭窄的相关性研究

Association between vasospasm during carotid artery stenting and instent restenosis after carotid artery stenting

目的分析颈动脉支架成形术中血管痉挛与术后再狭窄的相关性。方法收集2015年1月1日~2020年6月31日于台州市第一人民医院神经内科行支架置入治疗的颈动脉狭窄患者222例,根据术后1年内随访复查的影像资料是否发生支架内再狭窄(ISR)分为ISR组42例和无ISR组180例。比较2组临床资料和术中血管痉挛情况,采用二元logistic回归分析。结果ISR组饮酒(35.7%vs 18.3%,P=0.021)和术中血管痉挛比例较无ISR组更高(42.9%vs 15.6%,P=0.001)。二元logistic回归分析显示,术中血管痉挛与ISR的发生独立相关(OR=4.237,95%CI:1.971~9.106,P=0.000)。结论颈动脉支架成形术中血管痉挛与术后ISR相关。

Objective To analyze the associaation between vasospasm during carotid artery stenting(CAS) and instent restenosis after CAS.Methods Two hundred and twenty-two CAS patients who underwent stent implantation in Taizhou First People’s hospital from 2015-01-01 to 2020-06-31 were divided into instent restenosis(ISR) group(n=42) and ISR-free group(n=180) according to their follow-up imaging data within 1 year after stenting.The clinical data and incidence of vasospasm during CAS were compared between the two groups.The associaation between vasospasm during CAS and instent restenosis after CAS was analyzed by binary logistic regression analysis.Results The ratio of alcohol dinkers and incidence of vasospasm during CAS were significantly higher in ISR group than in ISR-free group(35.7% vs 18.3%,P=0.021;42.9% vs 15.6%,P=0.001).Binary logistic regression analysis showed that vasospasm during CAS was independently associated with ISR after CAS in carotid artery stenosis patients(OR=4.237,95%CI:1.971-9.106,P=0.000).Conclusion Vasospasm during CAS is associated with ISR after CAS in carotid artery stenosis patients.
